摘要
目的探讨单髁置换术(UKA)在老年膝关节内侧间室骨性关节炎患者中的应用价值.方法选择2019年2月—2021年2月该院收治的226例老年膝关节内侧间室骨性关节炎患者为研究对象,按照随机数字表法分为两组,每组113例.对照组采用全膝关节置换术(TKA),观察组采用UKA,两组术后均随访6个月.对比两组围术期相关指标、膝关节功能、膝关节活动度、并发症发生率、翻修率.结果观察组手术时间及住院时间均短于对照组,术中出血量少于对照组,组间差异有统计学意义(P<0.05).术后6个月,观察组美国纽约特种外科医院膝关节评分高于对照组,膝关节活动度大于对照组,组间差异有统计学意义(P<0.05).两组术后并发症发生率及术后随访6个月翻修率比较,组间差异无统计学意义(P>0.05).结论临床采用UKA治疗老年膝关节内侧间室骨性关节炎患者的疗效确切,手术时间短,且术中出血量少、创伤轻,能够有效提高患者的膝关节活动度,有利于其术后膝关节功能尽早恢复.
Objective To investigate the application value of unicompartmental knee arthroplasty(UKA)in elderly patients with osteoarthritis of medial compartment of the knee joint.Methods A total of 226 elderly patients with osteoarthritis of the medial compartment of the knee joint who were admitted to the hospital from February 2019 to February 2021 were selected as the research subjects,and divided into two groups according to the random number table method,with 113 cases in each group.Total knee arthroplasty(TKA)was used in the control group,and UKA was used in the observation group.Both groups were followed up for 6 months after operation.The perioperative related indicators,knee joint function,knee joint range of motion,complication rate and revision rate were compared between the two groups.Results The operation time and hospital stay of the observation group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group,and the differences between the groups were statistically significant(P<0.05).6 months after operation,the hospital for special surgery knee joint score of the observation group was higher than that of the control group,and the range of motion of the knee joint was greater than that of the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference between the two groups in the incidence of postoperative complications and the revision rate during the 6-month follow-up period after operation(P>0.05).Conclusion Clinical use of UKA in the treatment of elderly patients with osteoarthritis of the medial compartment of the knee joint has definite curative effect,with short operation time,less intraoperative blood loss and less trauma,which can effectively improve the range of motion of the knee joint of patients,and is conducive to the early recovery of knee joint function after operation.
