摘要
目的观察解郁降逆方、富马酸伏诺拉生片联合伊托必利片应用于胃食管反流病伴焦虑患者的临床效果.方法选取2020年1月—2022年1月高密市中医院脾胃病科收治的86例胃食管反流病伴焦虑患者,按照随机数字表法分为对照组和观察组,各43例.对照组口服富马酸伏诺拉生片联合伊托必利片,观察组在对照组基础上给予解郁降逆方.对比两组的总有效率、胃食管反流病问卷(GerdQ)评分、汉密尔顿焦虑量表(HAMA)评分、食管黏膜病变分级、血清中总抗氧化能力(T-AOC)、5-羟色胺(5-HT)水平、不良反应.结果观察组治疗总有效率90.70%(39/43)明显高于对照组的74.42%(32/43),差异有统计学意义(P<0.05).治疗4周后,观察组的GerdQ评分(7.89±1.23)分、HAMA评分(7.35±1.20)分均明显低于对照组,食管黏膜病变分级优于对照组,T-AOC水平(1.74±0.35)U/mg、5-HT水平(116.61±13.10)μg/L均明显高于对照组,组间差异有统计学意义(P<0.05).两组患者的不良反应发生率比较,差异无统计学意义(P>0.05).结论解郁降逆方、富马酸伏诺拉生片联合伊托必利片可有效改善胃食管反流病伴焦虑患者的临床症状,缓解焦虑,促进食管黏膜愈合,其起效可能与改善T-AOC、5-HT水平有关,安全可靠.
Objective To observe the clinical effect of Jieyu Jiangni prescription,vonoprazan fumarate tablets combined with itopride tablets in patients with gastroesophageal reflux disease with anxiety.Methods A total of 86 patients with gastroesophageal reflux disease and anxiety admitted to the Department of Spleen and Stomach Diseases,Gaomi Hospital of Traditional Chinese Medicine from January 2020 to January 2022 were selected and divided into a control group and an observation group according to the random number table method,with 43 cases in each group.The control group was given vonoprazan fumarate tablets combined with itopride tablets,and the observation group was given Jieyu Jiangni prescription on the basis of the control group.The total effective rate,gastroesophageal reflux disease questionnaire(GerdQ)score,Hamilton anxiety scale(HAMA)score,esophageal mucosal lesion grade,serum total antioxidant capacity(T-AOC),5-hydroxytryptamine(5-HT)level and adverse reactions were compared between the two groups.Results The total effective rate of treatment 90.70%(39/43)of the observation group was significantly higher than 74.42%(32/43)of the control group,and the difference was statistically significant(P<0.05).After 4 weeks of treatment,the GerdQ score(7.89±1.23)points and and HAMA score(7.35±1.20)points of the observation group were significantly lower than those of the control group,the grade of esophageal mucosal lesions in the observation group was better than that in the control group,the T-AOC level(1.74±0.35)U/mg and and 5-HT level(116.61±13.10)μg/L in the observation group were significantly higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The Jieyu Jiangni prescription,vonoprazan fumarate tablets combined with itopide tablets can effectively improve the clinical symptoms of patients with gastroesophageal reflux disease and anxiety,relieve anxiety,and promote the healing of esophageal mucosa.The effect may be related to improvement of TAOC and 5-HT levels,which is safe and reliable.
