摘要
目的探讨天舒胶囊联合氟桂利嗪治疗前庭性偏头痛的临床效果.方法选取2020年3月—2022年2月我院收治的88例前庭性偏头痛患者,按随机数字表法分为两组,各44例.对照组予以氟桂利嗪治疗,观察组在对照组基础上加用天舒胶囊治疗,均持续12周.比较两组临床疗效、偏头痛发作情况、脑血流速度、眩晕情况及不良反应.结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗12周后偏头痛发作次数(1.89±0.23)次/月少于对照组的(2.35±0.35)次/月,发作强度(1.89±0.23)分低于对照组的(2.47±0.35)分,发作时间(0.57±0.11)h短于对照组的(0.72±0.15)h,组间差异有统计学意义(P<0.05);观察组治疗12周后左侧大脑椎动脉平均血流速度(38.42±3.89)cm/s、基底动脉平均血流速度(43.45±4.27)cm/s及右侧大脑椎动脉平均血流速度(37.89±3.75)cm/s均慢于对照组的(43.15±4.07)cm/s、(47.89±4.32)cm/s、(42.41±3.87)cm/s,组间差异有统计学意义(P<0.05);观察组治疗12周后眩晕障碍量表中躯体评分(6.52±1.19)分、情绪评分(8.11±1.15)分、功能评分(8.28±1.18)分及总分(22.91±2.85)分均低于对照组的(7.87±1.22)分、(11.34±1.58)分、(10.57±1.24)分、(29.78±3.15)分,组间差异有统计学意义(P<0.05);两组不良反应发生率相比,差异无统计学意义(P>0.05).结论天舒胶囊联合氟桂利嗪可提高前庭性偏头痛治疗效果,缓解偏头痛发作,加快脑血流速度复常,改善眩晕,安全可靠.
Objective To investigate the clinical effect of Tianshu capsule combined with flunarizine in the treatment of vestibular migraine.Methods 88 patients with vestibular migraine admitted to our hospital from March 2020 to February 2022 were selected and divided into two groups according to the random number table method,with 44 patients in each group.The control group was treated with flunarizine,and the observation group was treated with Tianshu capsule on the basis of the control group,with both interventions for 12 weeks.The clinical efficacy,migraine attack,cerebral blood flow velocity,dizziness and adverse reactions were compared between the two groups.Results The total effective rate of treatment of the observation group was higher than that of the control group,with statistically significant differences between the groups(P<0.05);the number of migraine attacks(1.89±0.23)times/month of the observation group after 12 weeks of treatment was less than(2.35±0.35)times/month of the control group,the attack intensity(1.89±0.23)points was lower than(2.47±0.35)points of the control group,and the attack time(0.57±0.11)h was shorter than(0.72±0.15)h of the control group,with statistically significant differences between the groups(P<0.05);the mean blood flow velocities of left cerebral vertebral artery(38.42±3.89)cm/s,basilar artery(43.45±4.27)cm/s and right cerebral vertebral artery(37.89±3.75)cm/s in the observation group were slower than(43.15±4.07)cm/s,(47.89±4.32)cm/s,(42.41±3.87)cm/s in the control group,with statistically significant differences between the groups(P<0.05);the body score(6.52±1.19)points,mood score(8.11±1.15)points,function score(8.28±1.18)points and total score(22.91±2.85)points in dizziness handicap inventory in the observation group were lower than(7.87±1.22)points,(11.34±1.58)points,(10.57±1.24)points,(29.78±3.15)points in the control group,with statistically significant differences between the groups(P<0.05);there was no significant difference between the two groups(P<0.05).Conclusion Tianshu capsule combined with flunarizine can improve the therapeutic effect of vestibular migraine,relieve migraine attack,speed up the normalization of cerebral blood flow,and improve dizziness,which is safe and reliable.
