摘要
目的探讨输尿管硬镜联合NTrap阻石网篮碎石术在输尿管上段嵌顿性结石患者中的应用价值.方法选择2020年5月—2022年5月我院收治的输尿管上段嵌顿性结石患者74例,按随机数字表法分为两组.对照组37例采用经皮肾镜取石术,观察组37例采用输尿管硬镜联合NTrap阻石网篮碎石术.对比分析两组临床疗效、围术期相关指标、肾功能、术后并发症发生情况.结果观察组治疗总有效率94.59%高于对照组的75.68%,差异有统计学意义(P<0.05).观察组手术、碎石、住院时间均短于对照组,术后并发症发生率低于对照组,组间差异有统计学意义(P<0.05).观察组术后24 h血肌酐、血尿素、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)水平与术前相比,差异无统计学意义(P>0.05);但观察组术后24 h的NGAL水平低于对照组,差异有统计学意义(P<0.05).结论输尿管硬镜联合NTrap阻石网篮碎石术具有手术时间短、疗效确切的优势,能够有效促进输尿管上段嵌顿性结石患者术后恢复,减少术后并发症的发生,且能够保护患者肾功能,临床应用价值更高.
Objective To investigate the application value of ureteral rigidoscopy combined with NTrap lithotripsy mesh basket lithotripsy in patients with upper ureteral embedded stones.Methods 74 patients with embedded stones in the upper ureter admitted to our hospital from May 2020 to May 2022 were selected and divided into two groups according to the random number table method.In the control group,37 cases were treated with percutaneous nephrolithotomy,and in the observation group,37 cases were treated with ureteral rigidoscopy combined with NTrap lithotripsy mesh basket lithotripsy.The clinical efficacy,perioperative related indexes,renal function and postoperative complications of the two groups were compared and analyzed.Results The total effective rate of treatment in the observation group was 94.59%higher than 75.68%in the control group,and the difference was statistically significant(P<0.05).The operation,lithotripsy and hospitalization time of the observation group were shorter than those of the control group,and the incidence of postoperative complications was lower than that of the control group,and the differences between the groups were statistically significant(P<0.05).In the observation group,blood creatinine,blood urea and neutrophil gelatinase-associated lipocalin(NGAL)at 24 h after surgery were not statistically significant compared with those before surgery(P>0.05);but the NGAL at 24 h after surgery in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Ureteral rigidoscopy combined with NTrap lithotripsy mesh basket lithotripsy has the advantages of short operation time and precise efficacy,which can effectively promote the postoperative recovery of patients with upper ureteral incarcerated stones,reduce the occurrence of postoperative complications,and protect the renal function of patients,and has higher clinical application value.
