摘要
目的探讨穴位按摩联合中药封包热敷对老年全髋关节置换术(THA)术后康复的影响.方法选取2020年5月—2021年4月于泰安市中医二院行THA的老年患者76例为研究对象,按照随机数字表法分为对照组与研究组,每组38例.对照组术后进行常规康复功能训练,研究组在此基础上采用穴位按摩联合中药封包热敷.观察并比较两组患者干预前后的Harris髋关节评分量表(HHS)评分、疼痛视觉模拟评分(VAS)、Barthel指数(BI)评分,以及术后并发症情况.结果干预后,两组的HHS评分均高于干预前,且研究组评分高于对照组,组间差异有统计学意义(P<0.05).干预后,两组的VAS评分均低于干预前,且研究组评分低于对照组,组间差异有统计学意义(P<0.05).干预后,两组的BI评分均高于干预前,且研究组评分高于对照组,组间差异有统计学意义(P<0.05).研究组术后并发症发生率为5.26%,低于对照组的23.68%,差异有统计学意义(P<0.05).结论穴位按摩联合中药封包热敷干预能够有效促进老年THA术后患者髋关节功能恢复,减轻疼痛,降低并发症发生风险,提高日常生活活动能力,具有临床推广价值.
Objective To investigate the effect of acupoint massage combined with hot pot of traditional Chinese medicine on the postoperative rehabilitation of elderly patients with total hip arthroplasty(THA).Methods A total of 76 elderly patients receiving THA in the Second Hospital of Traditional Chinese Medicine of Tai'an City from May 2020 to April 2021 were selected as the research objects,and were divided into a control group and a study group according to random number table method,with 38 cases in each group.The control group received routine rehabilitation function training after surgery,and the study group received acupoint massage combined with traditional Chinese medicine hot pot on this basis.The Harris hip scale(HHS)score,pain visual analogue scale(VAS)score,Barthel index(BI)score before and after intervention,and the postoperative complications were observed and compared between the two groups.Results After intervention,the HHS scores of both groups was higher than before intervention,and the score of the study group was higher than that of the control group,the differences between groups were statistically significant(P<0.05).After intervention,the VAS scores in both groups were lower than before intervention,and the score in the study group was lower than that in the control group,the differences between groups were statistically significant(P<0.05).After intervention,the BI scores of both groups were higher than those before intervention,and the score of the study group was higher than that of the control group,the differences between the groups were statistically significant(P<0.05).The incidence of postoperative complications in the study group was 5.26%,lower than 23.68%in the control group,and the difference was statistically significant(P<0.05).Conclusion Acupoint massage combined with traditional Chinese medicine hot pot can effectively promote hip function recovery in elderly patients after THA,reduce pain and the risk of complications,and improve the ability of daily living activities,which has clinical promotion value.
