摘要
目的探讨微波疗法联合早期康复训练在下肢骨折患者中的应用效果.方法选取2020年7月—2022年2月我院收治的92例下肢骨折患者,按随机数字表法分为两组,各46例.对照组给予早期康复训练,观察组在对照组基础上采取微波疗法,持续干预4周.对比两组患肢肿痛、膝关节功能、骨折愈合时间、完全负重时间、并发症发生情况及生活质量.结果干预前,两组患肢肿胀值、视觉模拟评分(VAS)、国际膝关节文献委员会膝关节评估表(IKDC)评分、Lysholm膝关节功能评分量表(LKSS)评分、生活质量综合评定问卷(GQOLI-74)评分、术后骨折愈合时间、完全负重时间比较,组间差异无统计学意义(P>0.05).干预后,观察组患肢肿胀值(1.56±0.21)cm、VAS评分(1.67±0.28)分均低于对照组的(2.07±0.37)cm、(2.26±0.33)分,IKDC评分(84.06±4.43)分、LKSS评分(85.28±4.66)分均高于对照组的(76.23±6.01)分、(76.14±5.25)分,术后骨折愈合时间(4.58±0.68)周、完全负重时间(76.99±10.56)d均短于对照组的(5.13±0.44)周、(89.37±11.06)d,组间差异有统计学意义(P<0.05);随访3个月后,观察组GQOLI-74中的心理功能评分(82.16±6.70)分、躯体功能评分(86.21±6.63)分、社会功能评分(84.88±5.37)分、物质生活评分(85.01±5.25)分均高于对照组的(74.75±7.94)分、(74.97±5.40)分、(76.42±6.79)分、(76.67±5.84)分,组间差异有统计学意义(P<0.05).观察组并发症发生率为4.35%,低于对照组的19.57%,差异有统计学意义(P<0.05).结论微波疗法联合早期康复训练能加快下肢骨折患者患肢肿痛消退,改善膝关节功能,减少并发症的发生,利于骨折愈合及生活质量的提升.
Objective To investigate the application effect of microwave therapy combined with early rehabilitation training in patients with lower limb fracture.Methods 92 patients with lower limb fractures treated in our hospital from July 2020 to February 2022 were selected and divided into two groups according to the random number table method,with 46 cases in each group.The control group was given early rehabilitation training,and the observation group was given microwave therapy on the basis of the control group,with both interventions for 4 weeks.The two groups were compared in terms of affected limb swelling and pain,knee joint function,fracture healing time,total weight bearing time,occurrence of complications and quality of life.Results Before intervention,there were no significant differences in the affected limb swelling value,visual analog scale(VAS),International Knee Documentation Committee(IKDC)score,Lysholm knee score scale(LKSS)score,generic quality of life inventory-74(GQOLI-74)score,postoperative fracture healing time and total weight bearing time between the two groups(P>0.05).After intervention,the affected limb swelling value(1.56±0.21)cm and VAS score was(1.67±0.28)points in the observation group were lower than(2.07±0.37)cm and(2.26±0.33)points,the IKDC score(84.06±4.43)points and LKSS score(85.28±4.66)points were higher than(76.23±6.01)points and(76.14±5.25)points in the control group,postoperative fracture healing time(4.58±0.68)weeks and total weight bearing time(76.99±10.56)d were shorter than(5.13±0.44)weeks and(89.37±11.06)d in the control group,and the differences between the groups were statistically significant(P<0.05);after 3 months of follow-up,the psychological function score(82.16±6.70)points,physical function score(86.21±6.63)points,social function score(84.88±5.37)points and material life score(85.01±5.25)points in GQOLI-74 in the observation group were higher than(74.75±7.94)points,(74.97±5.40)points,(76.42±6.79)points and(76.67±5.84)points in the control group,and the differences between groups were statistically significant(P<0.05).The incidence of complications in the observation group was 4.35%,lower than 19.57%in the control group,and the difference was statistically significant(P<0.05).Conclusion Microwave therapy combined with early rehabilitation training can accelerate the regression of affected limb swelling and pain in patients with lower limb fracture,improve the knee joint function,reduce the occurrence of complications,and promote fracture healing and quality of life.
