达格列净联合甘精胰岛素对早期糖尿病肾病患者氧化应激反应、血管内皮功能及肾功能的影响 被引量:8

Effects of Dapagliflozin Combined with Insulin Glargine on Oxidative Stress Reaction,Vascular Endothelial Function and Renal Function in Patients with Early Diabetes Nephropathy

摘要 目的探究达格列净联合甘精胰岛素对早期糖尿病肾病(DN)患者氧化应激反应、血管内皮功能及肾功能的影响.方法选择2021年3月—2022年2月本院收治的86例早期DN患者,按照随机数字表法分为对照组与观察组,各43例.对照组予以甘精胰岛素治疗,观察组在对照组基础上联合达格列净治疗,均持续治疗3个月.比较两组临床疗效、氧化应激反应、血管内皮功能、肾功能、安全性.结果观察组治疗总有效率95.35%高于对照组的79.07%,差异有统计学意义(P<0.05).治疗前,两组超氧化物歧化酶(SOD)、丙二醛(MDA)、血管性血友病因子(vWF)、内皮素-1(ET-1)、尿素氮(BUN)、血肌酐(Scr)、尿白蛋白与尿肌酐比值(ACR)水平比较,组间差异无统计学意义(P>0.05);治疗后,观察组SOD水平(96.57±8.64)U/mL高于对照组,MDA水平(4.50±0.46)nmol/L、vWF水平(11.25±1.24)mg/L、ET-1水平(48.11±1.53)ng/L、BUN水平(6.53±0.56)mmol/L、Scr水平(87.22±3.28)μmol/L、ACR水平(161.13±8.94)mg·mmol/L均低于对照组,组间差异有统计学意义(P<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论达格列净联合甘精胰岛素治疗早期DN效果显著,能够有效减轻氧化应激反应,改善血管内皮功能及肾功能,且安全性高. Objective To investigate the effects of dapagliflozin combined with insulin glargine on oxidative stress reaction,vascular endothelial function and renal function in patients with early diabetes nephropathy(DN).Methods 86 patients with early DN admitted to the hospital from March 2021 to February 2022 were selected and divided into a control group and an observation group,with 43 patients in each group.The control group was treated with insulin glargine,and the observation group was treated with dapagliflozin on the basis of the control group,with both interventions for 3 months.The clinical efficacy,oxidative stress reaction,vascular endothelial function,renal function and safety of the two groups were compared.Results The total effective rate of treatment 95.35%of the observation group was significantly higher than 79.07%of the control group,and the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in levels of superoxide dismutase(SOD),malondialdehyde(MDA),von Willebrand factor(vWF),endothelin-1(ET-1),blood urea nitrogen(BUN),serum creatinine(Scr)and urinary albumin/creatinine ratio(ACR)between the two groups(P>0.05);after treatment,the SOD level(96.57±8.64)U/mL in the observation group was higher than that in the control group,MDA level(4.50±0.46)nmol/L,vWF level(11.25±1.24)mg/L,ET-1 level(48.11±1.53)ng/L,BUN level(6.53±0.56)mmol/L,Scr level(87.22±3.28)μmol/L and ACR level(161.13±8.94)mg·mmol/L were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Dapagliflozin combined with insulin glargine is effective in the treatment of early DN,which can effectively reduce oxidative stress reaction,improve vascular endothelial function and renal function,and has high safety.
