摘要
目的分析经尿道双极等离子体电切术治疗老年膀胱癌的效果.方法选取2020年4月—2022年4月本院收治的76例老年膀胱癌患者为研究对象,按随机数字表法分为两组,每组38例.对照组行膀胱部分切除术,观察组行经尿道双极等离子体电切术,均观察至术后3个月.对比两组患者的围手术期指标、生活质量及并发症发生情况.结果观察组的术中出血量为(43.54±3.69)mL,少于对照组的(56.79±4.57)mL,手术时间、导管留置时间、住院时间分别为(56.39±4.26)min、(4.71±1.06)d、(7.31±2.03)d,均短于对照组的(62.53±5.44)min、(6.54±1.79)d、(10.95±2.46)d,组间差异有统计学意义(P<0.05).术后,观察组的生活质量综合评定问卷内各维度评分均高于对照组,组间差异有统计学意义(P<0.05).观察组的并发症发生率为5.26%,低于对照组的21.05%,差异有统计学意义(P<0.05).结论采用经尿道双极等离子体电切术治疗老年膀胱癌的效果显著,可减少出血量,加快患者术后恢复,有效改善其生活质量,降低并发症发生率,值得临床推广使用.
Objective To analyze the effect of transurethral bipolar plasma electroresection on senile bladder cancer.Methods 76 elderly patients with bladder cancer admitted to the hospital from April 2020 to April 2022 were selected as the research objects and were divided into two groups according to random number table method,with 38 cases in each group.The control group underwent partial cystectomy,and the observation group underwent transurethral bipolar plasma electroresection.All the patients were observed until 3 months after surgery.The perioperative indexes,quality of life and complications were compared between the two groups.Results The intraoperative blood loss in the observation group was(43.54±3.69)mL,less than(56.79±4.57)mL in the control group,the operative time,catheter indindent time and hospital stay were(56.39±4.26)min,(4.71±1.06)d and(7.31±2.03)d,respectively,which were shorter than(62.53±5.44)min,(6.54±1.79)d and(10.95±2.46)d in the control group,and the differences between the groups were statistically significant(P<0.05).After surgery,the scores of all dimensions in the generic quality of life inventory-74 in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).The complication rate in the observation group was 5.26%,lower than 21.05%in the control group,and the difference was statistically significant(P<0.05).Conclusion Transurethral bipolar plasma electroresection is effective in the treatment of elderly patients with bladder cancer,which can reduce blood loss,accelerate postoperative recovery,effectively improve their quality of life,and reduce the incidence of complications.It is worthy of clinical application.
