镜像疗法与依达拉奉联合常规康复训练对脑卒中恢复期患者神经细胞因子及运动功能的影响

Effect of Mirror Image Therapy and Edaravone Combined with Conventional Rehabilitation Training on Neurocytokine and Motor Function in Patients with Stroke at Convalescent Stage

摘要 目的探讨镜像疗法与依达拉奉联合常规康复训练在脑卒中恢复期患者中的应用效果.方法选取2018年11月—2022年7月于我院接受治疗的102例脑卒中恢复期患者,采用随机数字表法分为两组,各56例.对照组在静脉滴注依达拉奉基础上接受常规康复训练,观察组在对照组基础上接受镜像疗法.比较两组治疗前与治疗1个月后的脑源性神经营养因子(BDNF)、神经源特异性烯醇化酶(NSE)、神经营养因子(NTF)水平、Fugl-Meyer运动功能评定量表(FMA)、卒中患者运动功能评估量表(MAS)评分,并记录治疗期间患者不良反应的发生情况.结果两组治疗前BDNF、NSE、NTF水平比较,组间差异无统计学意义(P>0.05);观察组治疗1个月后BDNF、NSE水平均较对照组低,NTF水平较对照组高,组间差异有统计学意义(P<0.05).两组治疗前FMA、MAS评分对比,组间差异无统计学意义(P>0.05);观察组治疗1个月后FMA、MAS评分均较对照组高,组间差异有统计学意义(P<0.05).观察组不良反应发生率为3.57%,与对照组的7.14%对比,差异无统计学意义(P>0.05).结论脑卒中恢复期患者经镜像疗法与依达拉奉联合常规康复训练治疗,对神经细胞因子水平具有一定调节效果,可改善运动功能,且具有一定安全性. Objective To explore the application effect of mirror image therapy and edaravone combined with conventional rehabilitation training in patients with stroke at convalescent stage.Methods 102 patients with stroke at convalescent stage who were treated in our hospital from November 2018 to July 2022 were selected and divided into two groups according to the random number table method,with 56 cases in each group.The control group received conventional rehabilitation training based on intravenous edaravone,and the observation group received mirror image therapy based on the control group.The brain-derived neurotrophic factor(BDNF),neurogenic specific enolase(NSE),neurotrophic factor(NTF)levels,Fugl-Meyer assessment(FMA),motor assessment scale(MAS)scores were compared between the two groups before treatment and after 1 month of treatment,and the occurrence of adverse reactions of patients during treatment were recorded.Results There were no statistically significant differences in the BDNF,NSE and NTF levels between the two groups before treatment(P>0.05);the BDNF and NSE levels in the observation group after 1 month of treatment were lower than those in the control group,and the NTF level was higher than that in the control group,with statistically significant differences between the groups(P<0.05).There were no statistically significant differences in FMA and MAS scores between the two groups before treatment(P>0.05);the FMA and MAS scores in the observation group after 1 month of treatment were higher than those in the control group,with statistically significant differences between the groups(P<0.05).The incidence of adverse reactions was 3.57%in the observation group,compared with 7.14%in the control group,there was no statistically significant difference(P>0.05).Conclusion The mirror image therapy and edaravone combined with conventional rehabilitation training in the patients with stroke at convalescent stage has a certain regulatory effect on the level of neurocytokines,can improve their motor function,and has a certain safety.
