摘要
目的探讨血必净联合纤维支气管镜(简称纤支镜)吸痰灌洗对老年重症肺炎(SP)患者血气分析及炎症反应的影响.方法选择2020年9月—2022年9月我院收治的老年SP患者72例,按随机数字表法分为两组.对照组(n=36)采用纤支镜吸痰灌洗联合盐酸氨溴索注射液治疗,观察组(n=36)采用纤支镜吸痰灌洗联合血必净治疗,两组均治疗7 d.对比两组血气分析指标、炎症反应、临床疗效、不良反应.结果治疗7 d后,观察组动脉血氧饱和度(93.76±3.02)%、动脉血氧分压(86.04±6.49)mmHg、治疗总有效率94.44%均高于对照组的(86.23±3.18)%、(78.85±5.27)mmHg、77.78%,动脉血二氧化碳分压(43.10±3.25)mmHg、C反应蛋白水平(9.16±1.53)mg/L、白细胞介素-6水平(25.19±3.16)pg/mL、肿瘤坏死因子α水平(48.24±4.20)ng/L、降钙素原水平(0.78±0.19)pg/L均低于对照组的(48.49±3.67)mmHg、(15.30±3.47)mg/L、(31.03±4.45)pg/mL、(63.11±5.76)ng/L、(1.53±0.28)pg/L,组间差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05).结论血必净联合纤支镜吸痰灌洗可显著改善老年SP患者血气分析及炎症指标,疗效显著,且安全性高.
Objective To investigate the effect of Xuebijing combined with sputum aspiration and lavage through fiberoptic bronchoscope on blood gas analysis and inflammatory reaction in elderly patients with severe pneumonia(SP).Methods 72 elderly patients with SP admitted to the hospital from September 2020 to September 2022 were selected and divided into two groups.The control group(n=36)was treated with sputum aspiration and lavage through fiberoptic bronchoscope combined with ambroxol hydrochloride injection,and the observation group(n=36)was treated with sputum aspiration and lavage through fiberoptic bronchoscope combined with Xuebijing,both lasted for 7 days.The blood gas analysis indicators,inflammatory reaction,clinical efficacy and adverse reactions were compared between the two groups.Results After 7 days of the treatment,the arterial oxygen saturation(93.76±3.02)%,arterial partial pressure of oxygen(86.04±6.49)mmHg,the total effective rate of treatment 94.44%in the observation group were higher than(86.23±3.18)%,(78.85±5.27)mmHg,77.78%in the control group,while the arterial partial pressure of carbon dioxide(43.10±3.25)mmHg,C-reactive protein level(9.16±1.53)mg/L,interleukin-6 level(25.19±3.16)pg/mL,tumor necrosis factorαlevel(48.24±4.20)ng/L,procalcitonin level(0.78±0.19)pg/L were lower than(48.49±3.67)mmHg,(15.30±3.47)mg/L,(31.03±4.45)pg/mL,(63.11±5.76)ng/L,(1.53±0.28)pg/L in the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of adverse reactions of the two groups were compared,and the difference was not statistically significant(P>0.05).Conclusion Xuebijing combined with sputum aspiration and lavage through fiberoptic bronchoscope can better improve the blood gas analysis and inflammatory indicators in elderly patients with SP,with significant efficacy and high safety.
