摘要
目的分析采用经跗骨窦切口微创钢板结合空心钉内固定术治疗跟骨骨折的效果.方法选取2019年1月—2022年1月本院收治的80例跟骨骨折患者,按随机数字表法分为两组,每组40例.对照组行外侧"L"形切口内固定术,观察组行经跗骨窦切口微创钢板结合空心钉内固定术,均观察至术后6个月.对比两组患者的手术相关指标、足踝功能、生活质量及并发症发生情况.结果观察组的术中出血量为(30.43±2.69)mL,少于对照组的(60.71±4.89)mL,手术时间、消肿时间、愈合拆线时间分别为(69.31±5.71)min、(2.06±0.31)d、(13.49±2.03)d,均短于对照组的(83.26±7.04)min、(3.49±0.65)d、(18.71±2.58)d,组间差异有统计学意义(P<0.05).术后6个月,观察组的美国足踝外科协会踝关节评分及生活质量综合评定问卷中各维度评分均高于对照组,组间差异有统计学意义(P<0.05).观察组的并发症发生率为5.00%,低于对照组的20.00%,差异有统计学意义(P<0.05).结论采用经跗骨窦切口微创钢板结合空心钉内固定术治疗跟骨骨折的效果理想,可减少出血量,缩短操作时间,加速患者术后足踝功能恢复,改善其生活质量,且并发症较少,临床应用价值较高.
Objective To analyze the effect of minimally invasive plate fixation and hollow nail internal via tarsal sinus incision on calcaneal fracture patients.Methods 80 patients with calcaneal fracture admitted to the hospital from January 2019 to January 2022 were selected and were randomly divided into two groups according to random number table method,with 40 patients in each group.The control group received internal fixation through the lateral"L"shaped incision,and the observation group received minimally invasive plate fixation and hollow nail internal via tarsal sinus incision.Both groups were observed until 6 months after surgery.The operation related indexes,foot and ankle function,quality of life and complications were compared between the two groups.Results The intraoperative blood loss in the observation group was(30.43±2.69)mL,which was less than(60.71±4.89)mL in the control group,the operative time,detumescence time and healing suture removal time were(69.31±5.71)min,(2.06±0.31)d and(13.49±2.03)d,respectively,which were shorter than(83.26±7.04)min,(3.49±0.65)d and(18.71±2.58)d in the control group,and the differences between the groups were statistically significant(P<0.05).6 months after surgery,the ankle joint score of the American Orthopaedic Foot and Ankle Society and the scores of all dimensions in the generic quality of life inventory-74 in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of complications in the observation group was 5.00%,lower than 20.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion The treatment of calcaneal fracture with minimally invasive plate fixation and hollow nail internal via tarsal sinus incision has ideal effect,which can reduce the amount of blood loss,shorten the operation time,accelerate the postoperative functional recovery of the foot and ankle,improve the quality of life of patients with fewer complications,and has high clinical application value.
