功能性电刺激联合踏车训练对脑卒中后偏瘫肩痛患者的康复疗效观察

Observation on Rehabilitation Effect of Functional Electrical Stimulation Combined with Cycling Training in Patients with Hemiplegic Shoulder Pain after Stroke

摘要 目的探究功能性电刺激(FES)联合踏车训练在脑卒中后偏瘫肩痛患者中的应用效果.方法选取2021年1月—2022年10月本院收治的40例脑卒中后偏瘫肩痛患者为研究对象,根据随机数字表法分为两组.两组患者均接受临床药物治疗和常规的综合康复治疗,对照组(n=20)采用FES治疗,试验组(n=20)在对照组基础上联合踏车训练.比较两组患者治疗前和治疗6周后肩关节疼痛程度、上肢运动功能、日常生活能力.结果治疗前,两组疼痛视觉模拟(VAS)评分、Fugl-Meyer运动功能评定量表(FMA)上肢部分评分、改良Barthel指数(MBI)评分比较,组间差异无统计学意义(P>0.05);治疗6周后,试验组VAS评分(1.75±0.94)分低于对照组的(2.75±0.91)分,FMA上肢部分评分(22.30±6.01)分、MBI评分(68.25±9.78)分均高于对照组的(19.30±7.04)分、(58.75±12.83)分,组间差异有统计学意义(P<0.05).结论脑卒中后偏瘫肩痛患者采用FES联合踏车训练能有效改善患者的肩痛及上肢运动功能,提高其日常生活能力. Objective To explore the application effect of functional electrical stimulation(FES)combined with cycling training in patients with hemiplegic shoulder pain after stroke.Methods 40 patients with hemiplegic shoulder pain after stroke who were admitted to the hospital from January 2021 to October 2022 were selected as the research objects,and they were divided into two groups according to the random number table method.Both groups received clinical drug therapy and conventional comprehensive rehabilitation therapy,the control group(n=20)used FES,and the experimental group(n=20)used cycling training on the basis of the control group.The degree of shoulder pain,upper motor function and activities of daily living of the two groups of patients before treatment and after 6 weeks of treatment were compared.Results Before treatment,there were no statistically significant differences in the pain visual analogue scale(VAS)score,upper limb of Fugl-Meyer assessment(FMA)score and modified Barthel index(MBI)score between the two groups(P>0.05);after 6 weeks of treatment,VAS score(1.75±0.94)points of the experimental group was lower than(2.75±0.91)points of the control group,upper limb of FMA score(22.30±6.01)points and MBI score(68.25±9.78)points were higher than(19.30±7.04)points and(58.75±12.83)points of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The FES combined with cycling training for patients with hemiplegic shoulder pain after stroke can effectively improve the patient's shoulder pain,upper limb motor function and ability of daily living.
