摘要
目的探究改良型呼吸机管道固定方法对经鼻持续正压通气(NCPAP)新生儿血气水平、通气时间及睡眠时间的影响。方法选取本院2021年1月—2022年12月收治的116例新生儿NCPAP患儿为研究对象,随机分为两组。对照组(n=58)采用常规固定方法,观察组(n=58)采用改良型呼吸机管道固定帽及固定方法。对比两组患儿的血气指标、通气时间、睡眠时间、并发症及不良反应发生情况。结果干预后,观察组的血氧分压、血氧饱和度均高于对照组,二氧化碳分压低于对照组,但组间差异无统计学意义(P>0.05)。干预后,观察组的日均睡眠时间为(17.36±1.34)h,长于对照组的(16.54±1.62)h,NCPAP通气时间为(76.49±5.69)h,短于对照组的(81.66±6.34)h,组间差异有统计学意义(P<0.05)。观察组的鼻中潮红、鼻周压红、呼吸机管道脱落、鼻塞漏气、面颊部皮肤压痕、额头部皮肤压痕发生率均低于对照组,但组间差异无统计学意义(P>0.05)。结论改良型呼吸机管道固定方法应用于NCPAP新生儿中的效果确切,可缩短其通气时间,增加睡眠时间,并可在一定程度上改善其血气指标,值得临床推广使用。
Objective To investigate the effects of modified ventilator tube fixation on blood gas level,ventilation time and sleep time of neonates with nasal continuous positive airway pressure(NCPAP).Methods 116 neonates with NCPAP admitted to the hospital from January 2021 to December 2022 were selected as the study objects and were randomly divided into two groups.The control group(n=58)were fixed by conventional method,and the observation group(n=58)were fixed by modified ventilator tube cap and fixing method.The blood gas indexes,ventilation time,sleep time,complications and adverse reactions were compared between the two groups.Results After the intervention,the blood oxygen partial pressure and blood oxygen saturation of the observation group were higher than those of the control group,and the carbon dioxide fraction was lower than that of the control group,but there were no statistical significances between the groups(P>0.05).After the intervention,the average daily sleep time of the observation group was(17.36±1.34)h,longer than(16.54±1.62)h of the control group,and the NCPAP ventilation time was(76.49±5.69)h,shorter than(81.66±6.34)h of the control group,the differences between the groups were statistically significant(P<0.05).The incidences of nasal flushing,perinasal pressure,ventilator tube loss,nasal congestion and air leakage,cheek skin indentation and forehead skin indentation in observation group were lower than those in control group,but there were no statistical significances between the groups(P>0.05).Conclusion The modified ventilator tube fixation method is effective in NCPAP neonates,which can shorten the ventilation time,increase the sleep time,and improve the blood gas indexes to a certain extent,and is worthy of clinical promotion.
