摘要
目的探讨温通刮痧在全膝关节置换术(TKA)后气滞血瘀型患者中对下肢肿痛中的疗效。方法选取2022年1月—2023年5月深圳平乐骨伤科医院收治并接受TKA的60例患者作为研究对象,按随机数字表法分为两组。对照组(n=30)采取常规护理,观察组(n=30)在对照组基础上加用温通刮痧。对比两组患者护理前后的炎症因子、疼痛肿胀、膝关节功能。结果术前,两组炎症因子、疼痛肿胀程度、膝关节功能评分比较,组间差异无统计学意义(P>0.05)。术后3、7、14 d,观察组白细胞介素-6水平分别为(3.54±0.76)ng/L、(2.43±0.60)ng/L、(1.39±0.36)ng/L,C反应蛋白水平分别为(47.22±9.13)mg/L、(28.40±5.16)mg/L、(17.01±3.71)mg/L,疼痛视觉模拟评分分别为(2.67±0.55)分、(2.33±0.48)分、(2.03±0.18)分,均低于对照组,肢体周径均小于对照组,膝关节功能评分均高于对照组,组间差异有统计学意义(P<0.05)。结论TKA术后患者中发生膝关节疼痛、肿胀时,基于常规护理的温通刮痧疗法可加速关节功能康复进程。
Objective To explore the effect of wentong scrapping therapy on lower limb swelling and pain in patients with qi stagnation and blood stasis after total knee arthroplasty(TKA).Methods Sixty patients who were admitted to Shenzhen Pingle Orthopedic Hospital received TKA from January 2022 to May 2023 were included in this study followed by being divided into two groups using a random number table.The control group(n=30)was given routine nursing care,while the experimental group(n=30)received wentong scrapping therapy in addition to the routine care provided to the control group.The inflammatory factors,pain and swelling,and knee joint function before and after nursing care in the two groups of patients were compared.Results Before operation,there was no significant difference in inflammatory factors,pain and swelling degree,and knee joint function score between the two groups(P>0.05).3,7 and 14 days after operation,the levels of interleukin-6 in the observation group were(3.54±0.76)ng/L,(2.43±0.60)ng/L and(1.39±0.36)ng/L,respectively,the levels of C-reactive protein were(47.22±9.13)mg/L,(28.40±5.16)mg/L,(17.01±3.71)mg/L,and the pain Visual Analogue Scale scores were(2.67±0.55)points,(2.33±0.48)points,(2.03±0.18)points,respectively,the limb circumference of the experimental group was smaller than that of the control group,and the knee joint function score was higher than that of the control group,the differences between the two groups were statistically significant(P<0.05).Conclusion The use of wentong scrapping therapy,based on routine nursing care,can accelerate the recovery process of joint function in patients experiencing knee joint pain and swelling after TKA surgery.
