摘要
目的探讨加味四妙汤联合依托考昔对急性痛风性关节炎患者中医证候和关节功能的影响。方法选择2022年3月—2023年3月宝鸡第三医院收治的80例急性痛风性关节炎患者为研究对象,随机分为对照组和观察组。对照组(n=40)采用依托考昔治疗,观察组(n=40)采用加味四妙汤联合依托考昔治疗。比较两组临床疗效差异,中医症候评分,Lysholm膝关节评分量表(LKSS)评分及不良反应发生情况。结果观察组的临床总有效率为92.50%,高于对照组的72.50%,差异有统计学意义(P<0.05)。治疗前,两组的关节疼痛、活动受限和红肿评分比较,组间差异无统计学意义(P>0.05);治疗后,两组的关节疼痛、活动受限和红肿评分均较治疗前降低,且观察组关节疼痛、活动受限和红肿评分均低于对照组,差异有统计学意义(P<0.05)。治疗前,两组的LKSS评分比较,差异无统计学意义(P>0.05);治疗后,两组的LKSS评分均较治疗前升高,且观察组LKSS评分高于对照组,差异有统计学意义(P<0.05)。观察组发生不良反应2例,对照组发生不良反应1例,主要表现为恶心纳差和胃脘部不适等,不需要停药,给予对症处理后症状基本消失。结论加味四妙汤联合依托考昔对急性痛风性关节炎具有较好的临床疗效,能明显改善患者的中医证候和关节功能。
Objective To study the effect of Jiawei Simiao Decoction combined with etokoxi on traditional Chinese medicine(TCM)syndromes and joint function in patients with acute gouty arthritis.Methods 80 cases with acute gouty arthritis were selected as the research objects from Baoji Third Hospital from March 2022 to March 2023,which were divided into a control group and observation group randomly.40 cases in control group were treated with etokoxi,40 cases in observation group were treated with Jiawei Simiao Decoction combined with etocoxime.The curative effect,TCM syndrome score,Lyshom Knee Score Scale(LKSS)score and adverse reactions of the two groups were compared.Results The total clinical effective rate of observation group was 92.50%,which was higher than 72.50%of control group,the difference was not statistically significant(P>0.05).Before treatment,there was no significant difference in joint pain,mobility limitation and redness scores between the two groups(P>0.05);after treatment,joint pain,mobility limitation and redness scores in both groups were lower than before treatment,and the scores of joint pain,mobility limitation and redness in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Before treatment,there was no significant difference in LKSS scores between the two groups(P>0.05);after treatment,LKSS scores in both groups were higher than those before treatment,and the LKSS score of the observation group was higher than that of the control group,and the differences were statistically significant(P<0.05).Two cases of adverse reactions occurred in the observation group,and one case in the control group,which is mainly manifested as poor nausea and gastric duct discomfort,etc.There was no need to stop the drug,and the symptoms basically disappeared after symptomatic treatment.Conclusion The combination of Jiawei Simiao Decoction and Etocoxib has a good therapeutic effect on acute gouty arthritis,which can significantly improve TCM syndrome score and joint function.
