摘要
目的探讨早期部分负重训练联合本体感觉训练对膝关节骨折手术患者平衡功能的影响.方法选择2020年1月—2022年8月珠海市第五人民医院收治的102例膝关节骨折手术患者为研究对象,按随机数字表法将其分为对照组和观察组,各51例.对照组采用本体感觉训练,观察组采用早期部分负重训练联合本体感觉训练,两组均训练24周.比较两组患者的临床疗效、平衡功能、膝关节功能、步行能力、疼痛视觉模拟评分(VAS)和关节活动度.结果观察组治疗总有效率为94.12%,高于对照组的80.39%,差异有统计学意义(P<0.05);治疗后,观察组足底重心压力移动距离为(295.63±12.84)mm,短于对照组的(403.54±14.16)mm,移动轨迹面积为(465.98±15.93)mm^(2),小于对照组的(695.35±20.51)mm^(2),观察组Lysholm评分、Tinetti量表评分分别为(90.14±9.80)分、(25.03±2.29)分,均高于对照组的(71.69±9.03)分、(18.01±1.95)分,观察组VAS评分为(1.85±0.39)分,低于对照组的(2.47±0.54)分,关节活动度为(105.03±17.42)°,大于对照组的(74.86±15.13)°,组间差异有统计学意义(P<0.05).结论早期部分负重训练联合本体感觉训练对膝关节骨折手术患者效果确切,可有效改善患者平衡功能及膝关节功能,安全可靠.
Objective To explore the effect of early partial weight-bearing training combined with proprioceptive training on balance function in patients undergoing knee fracture surgery.Methods 102 patients with knee fracture surgery admitted to Zhuhai Fifth People's Hospital from January 2020 to August 2022 were selected as the research objects and randomly divided into control group and observation group,with 51 patients in each group.The control group received proprioceptive training,and the observation group received early partial weight training combined with proprioceptive training.Both groups were trained for 24 weeks.The clinical efficacy,balance function,knee function,walking ability,pain Visual Analog Scale(VAS)and joint motion were compared between the two groups.Results The total effective rate of the observation group was 94.12%,higher than 80.39%of the control group,and the difference was statistically significant(P<0.05);after treatment,the movement distance of the plantar center of gravity pressure in the observation group was(295.63±12.84)mm,which was shorter than(403.54±14.16)mm in the control group,the movement trajectory area was(465.98±15.93)mm^(2),which was smaller than(695.35±20.51)mm^(2) in the control group,the Lysholm score and Tinetti scale score in the observation group were(90.14±9.80)points and(25.03±2.29)points,respectively,which were higher than(71.69±9.03)points and(18.01±1.95)points in the control group;VAS score of observation group was(1.85±0.39)points,which was lower than(2.47±0.54)points of control group;the range of motion of the joint was(105.03±17.42)°,which was higher than(74.86±15.13)°of the control group,and the differences between groups were statistically significant(P<0.05).Conclusion Early partial weight-bearing training combined with proprioception training has a definite effect on patients undergoing knee fracture surgery,which can effectively improve their balance function and knee joint function,and is safe and reliable.
