摘要
目的探讨TMS前庭平衡投射点结合强化前庭感觉整合训练对脑卒中偏瘫患者平衡功能及运动能力的影响.方法选择2021年9月—2023年3月广西医科大学第二附属医院收治的116例脑卒中偏瘫患者为研究对象,按随机数字表法将其分为对照组和观察组,各58例.对照组采用强化前庭感觉整合训练,观察组在对照组基础上采用TMS前庭反射进行干预.比较两组患者腿部肌张力、日常生活能力、平衡功能及运动能力.结果干预后,观察组的改良肌张力Ashworth评估量表评分为(1.54±0.30)分,低于对照组,巴氏量表、Berg平衡量表、Fugl-Meyer运动功能评定量表-上肢功能、Fugl-Meyer运动功能评定量表-下肢功能评分分别为(86.39±6.43)分、(45.57±5.62)分、(52.51±4.24)分、(27.29±2.79)分,均高于对照组,组间差异有统计学意义(P<0.05).结论TMS前庭平衡投射点结合强化前庭感觉整合训练干预护理脑卒中偏瘫平衡患者,疗效确切,可降低患者异常的肌张力,增强其日常生活能力,改善平衡及运动能力.
Objective To explore the effect of TMS vestibular balance projection point combined with enhanced vestibular sensory integration training on balance function and motor ability in stroke patients with hemiplegia.Methods 116 stroke patients with hemiplegia who were admitted to the Second Affiliated Hospital of Guangxi Medical University from September 2021 to March 2023 were selected as the research objects.According to the random number table method,they were divided into a control group and an observation group,with 58 cases in each group.The control group was treated with enhanced vestibular sensory integration training,and the observation group was treated with TMS vestibular reflex intervention on the basis of the control group.The leg muscle tension,daily living ability,balance function and motor ability were compared between the two groups.Results After intervention,the Modified Ashworth Scale score of the observation group was(1.54±0.30)points,lower than that of the control group,and the Barthel Index,Berg Balance Scale,the Fugl-Meyer Assessment of the Upper Extremity and Lower Extremity scores were(86.39±6.43)points,(45.57±5.62)points,(52.51±4.24)points and(27.29±2.79)points,respectively,higher than those of the control group,the difference between the two groups was statistically significant(P<0.05).Conclusion TMS vestibular balance projection point combined with enhanced vestibular sensory integration training intervention nursing for stroke hemiplegic balance patients has a definite therapeutic effect,which can reduce abnormal muscle tone,enhance their daily living ability,and improve balance and motor ability.
