摘要
目的探讨天麻钩藤饮加减联合盐酸氟桂利嗪治疗肝阳上亢型偏头痛的临床效果.方法选择济宁医学院附属金乡医院2020年12月—2022年12月收治的126例肝阳上亢型偏头痛患者为研究对象,以随机数表法将其分为对照组及观察组.对照组(n=63)采用盐酸氟桂利嗪治疗,观察组(n=63)在此基础上采用天麻钩藤饮加减治疗,均持续治疗4周.比较两组患者的临床疗效、发作次数、持续时间、疼痛程度及生活质量.结果观察组的治疗总有效率为95.24%,高于对照组的80.95%,差异有统计学意义(P<0.05).治疗后,观察组的发作次数为(3.10±0.86)次/月,少于对照组,持续时间为(2.58±0.81)h/次,短于对照组,疼痛视觉模拟评分为(2.08±0.58)分,低于对照组,组间差异有统计学意义(P<0.05);观察组的偏头痛生活品质问卷中功能限制、功能障碍、情感功能评分分别为(78.41±6.15)分、(85.20±6.51)分、(84.20±5.76)分,均高于对照组,组间差异有统计学意义(P<0.05).结论肝阳上亢型偏头痛患者采用盐酸氟桂利嗪联合天麻钩藤饮加减治疗能提高疗效,改善头痛情况,提升生活质量.
Objective To investigate the clinical effect of modified Tianma Guteng Decoction combined with flunarizine hydrochloride in the treatment of hyperactive liver-Yang type migraine.Methods 126 patients with hyperactive liver-Yang type migraine admitted to Jinxiang Hospital,Affiliated Hospital of Jining Medical University from December 2020 to December 2022 were selected as the study objects and were divided into a control group and an observation group by random number table method.The control group(n=63)was treated with flunarizine hydrochloride,and the observation group(n=63)was treated with modified Tianma Guteng Decoction on this basis.Both groups were treated for 4 weeks.The treatment effect,frequency of attacks,duration,pain degree and quality of life were compared between the two groups.Results The total effective rate of observation group was 95.24%,higher than 80.95%of control group,the difference was statistically significant(P<0.05).After treatment,the number of attacks in the observation group was(3.10±0.86)times/month,less than that in the control group,the duration was(2.58±0.81)h/time,shorter than that in the control group,and the Visual Analogue Scale score was(2.08±0.58)points,lower than that in the control group,the differences between the groups were statistically significant(P<0.05);the scores of functional limitation,functional impairment and emotional function in the migraine quality of life questionnaire of the observation group were(78.41±6.15)points,(85.20±6.51)points and(84.20±5.76)points,respectively,which were higher than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Flunarizine hydrochloride combined with modified Tianma Guteng Decoction can improve the curative effect,release the headache and improve the quality of life in patients with hyperactive liver-Yang migraine.
