摘要
目的探讨俞募配穴针刺法辅助补阳还五汤治疗中风后气虚血瘀型便秘的临床效果。方法选取我院2020年5月—2023年5月收治的84例中风后气虚血瘀型便秘患者为研究对象,按随机数字表法分为对照组观察组,各42例。对照组采用补阳还五汤治疗,观察组在对照组基础上联合俞募配穴针刺法治疗,两组均持续治疗2周。比较两组患者的临床疗效、排便情况、中医证候积分、胃肠激素水平及不良反应发生情况。结果观察组治疗总有效率为95.24%,高于对照组的80.95%,差异有统计学意义(P<0.05)。治疗2周后,观察组排便困难程度、排便频率、粪便性状、排便不尽感评分均低于对照组,组间差异有统计学意义(P<0.05);观察组少腹胀急、胃纳减退、面色淡白、气短乏力积分均低于对照组,组间差异有统计学意义(P<0.05);观察组MTL、VIP、SP水平均高于对照组,组间差异有统计学意义(P<0.05)。两组不良反应相比,差异无统计学意义(P>0.05)。结论俞募配穴针刺法辅助补阳还五汤可提高中风后气虚血瘀型便秘治疗效果,纠正异常排便情况,改善胃肠激素水平,降低中医证候积分,安全可靠。
Objective To investigate the clinical effect of Shu-mu combined acupuncture with Buyang Huanwu Decoction in the treatment of constipation with Qi deficiency and blood stasis syndrome after stroke.Methods 84 patients with post-stroke Qi-deficiency and blood-stasis constipation admitted to our hospital from May 2020 to May 2023 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 42 cases in each group.The control group was treated with Buyang Huanwu decoction,and the observation group was treated with Shu Mu combined with acupuncture on the basis of the control group.Both groups were treated for 2 weeks.The clinical efficacy,defecation,TCM syndrome score,gastrointestinal hormone level and adverse reactions were compared between the two groups.Results The total effective rate of observation group was 95.24%,higher than 80.95%of the control group,the difference was statistically significant(P<0.05).After 2 weeks of treatment,the defecation difficulty,defecation frequency,fecal character and defecation feeling score in observation group were lower than those in control group,and the differences between groups were statistically significant(P<0.05);the scores of acute abdominal distension,hypoglycemia,pale complexion and shortness of breath in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05);the levels of MTL,VIP and SP in the observation group were higher than those in the control group,and the differences between groups were statistically significant(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Shu-Mu combined with acupoint acupuncture assisted Buyang Huanwu Decoction can improve the treatment effect of constipation with Qi deficiency and blood stasis after stroke,correct abnormal defecation,improve gastrointestinal hormone level and reduce TCM syndrome score,which is safe and reliable.
