摘要
目的探讨在针对乳腺癌(BC)患者手术后伴有上肢淋巴水肿时采用五皮饮辅助温针灸治疗的效果。方法选取2020年1月—2022年1月本院收治的60例BC术后上肢淋巴水肿患者为研究对象,按随机数字表法将其分为对照组与观察组,各30例。其中对照组术后采用温针灸治疗,观察组在此基础上采用五皮饮治疗。比较两组患肢周径、上肢患-健侧周径差、中医证候积分及生活质量。结果治疗后,观察组患肢周径中的上臂围(26.53±3.21)cm、前臂围(24.40±2.98)cm及上肢患-健侧周径差(2.44±0.31)cm均小于对照组的(28.44±3.21)cm、(26.24±3.01)cm、(3.32±0.51)cm,组间差异有统计学意义(P<0.05)。观察组肢体疼痛评分(1.08±0.17)分、肢体水肿评分(0.75±0.22)分、肢体麻木评分(0.85±0.24)分、疲倦乏力评分(0.73±0.18)分及皮肤紧绷评分(0.48±0.13)分均低于对照组的(1.68±0.18)分、(1.43±0.24)分、(1.54±0.25)分、(1.14±0.13)分、(1.46±0.40)分,组间差异有统计学意义(P<0.05)。观察组BC生命质量量表评分(108.68±14.15)分高于对照组的(101.36±12.19)分,差异有统计学意义(P<0.05)。结论五皮饮结合温针灸治疗效果显著,可有效缩减BC术后上肢淋巴水肿患者的患肢周径及上肢患-健侧周径差,缓解临床症状,并提高生活质量,值得推广。
Objective To investigate the effect of Wupiyin assisted warm acupuncture treatment for breast cancer(BC)patients with upper limb lymphedema after operation.Methods 60 patients with upper limb lymphedema after breast cancer surgery admitted to our hospital from January 2020 to January 2022 were selected as the research objects.According to the random number table method,they were divided into a control group and an observation group,with 30 cases in each group.The control group was treated with warm acupuncture after operation,and the observation group was treated with Wupi decoction on the basis of the control group.The circumference of the affected limb,the difference between the affected and healthy sides of the upper limb,the TCM syndrome score and the quality of life were compared between the two groups.Results After treatment,The upper arm circumference(26.53±3.21)cm,forearm circumference(24.40±2.98)cm and the circumference difference between the affected and healthy side of the upper limb(2.44±0.31)cm in the observation group were lower than(28.44±3.21)cm,(26.24±3.01)cm,(3.32±0.51)cm in the control group,the differences between the two groups were statistically significant(P<0.05).The limb pain score(1.08±0.17)points,limb edema score(0.75±0.22)points,limb numbness score(0.85±0.24)points,fatigue score(0.73±0.18)points and skin tightness score(0.48±0.13)points in the observation group were lower than(1.68±0.18)points,(1.43±0.24)points,(1.54±0.25)points,(1.14±0.13)points,(1.46±0.40)points in the control group,the differences between the two groups were statistically significant(P<0.05).The score of breast Functional Assessment of Cancer Therapy Scale in the observation group was(108.68±14.15)points,which was higher than(101.36±12.19)points in the control group,and the difference was statistically significant(P<0.05).Conclusion Wupiyin combined with warm acupuncture has remarkable effect,which can effectively reduce the circumference difference of the affected limb and the circumference difference between the affected and healthy side of the upper limb in patients with upper limb lymphedema after breast cancer surgery,alleviate clinical symptoms,and improve the quality of life,which is worth promoting.
