摘要
目的探讨错配修复基因(MMR)和预后营养指数(PNI)对结直肠癌患者术后复发及转移事件的预测价值。方法回顾性分析2018年1月—至2020年12月于江门市五邑中医院接受根治性切除术的110例结直肠癌患者的病例资料,根据随访期内复发及转移情况将其分为对照组及复发转移组,比较两组患者的临床资料及MMR、PNI水平,以多因素Logistic回归模型验证MMR和PNI对结直肠癌术后复发及转移的影响。结果110例结直肠癌患者中,共有52例出现术后复发或转移,复发转移率为47.27%。对照组与复发转移组患者的肿瘤最大径、MMR状态、PNI水平比较,组间差异有统计学意义(P<0.05)。多因素Logistic回归结果表明,在校正了肿瘤家族史、肿瘤最大径、组织学类型等混杂因素后,错配修复正常(pMMR)状态(OR=2.659,95%CI:1.415~4.998)和低PNI水平(OR=2.25,95%CI:1.370~3.709)是结直肠癌患者术后复发转移的独立危险因素(P<0.05)。结论MMR和PNI水平是结直肠癌患者术后复发转移的影响因素,pMMR和低PNI人群具有更高的肿瘤复发转移风险。
Objective To investigate the predictive value of mismatch repair gene(MMR)and prognostic nutrition index(PNI)in postoperative recurrence and metastasis of colorectal cancer patients.Methods The case data of 110 patients with colorectal cancer who received radical resection in Wuyi Hospital of Traditional Chinese Medicine,Jiangmen City from January 2018 to December 2020 were retrospectively analyzed.According to the recurrence and metastasis during the follow-up period,they were divided into a control group and a recurrence and metastasis group.The clinical data and the levels of MMR and PNI were compared between the two groups,and the effects of MMR and PNI on postoperative recurrence and metastasis of colorectal cancer were verified by multivariate Logistic regression model.Results Among 110 patients with colorectal cancer,52 cases had recurrence or metastasis after operation,and the recurrence and metastasis rate was 47.27%.There were significant differences in tumor maximum diameter,MMR status and PNI level between the control group and the recurrence and metastasis group(P<0.05).Multivariate Logistic regression results showed that proficient mismatch repair(pMMR)status(OR=2.659,95%CI:1.415~4.998)and low PNI level(OR=2.25,95%CI:1.370~3.709)after adjusting for confounding factors such as family history of tumor,maximum tumor diameter,histological type,etc.were independent risk factors for postoperative recurrence and metastasis in colorectal cancer patients(P<0.05).Conclusion The levels of MMR and PNI are the influential factors for postoperative recurrence and metastasis of colorectal cancer patients,pMMR and low PNI population have a higher risk of tumor recurrence and metastasis.
