摘要
目的分析黄芪桂枝五物汤联合四肢联动训练在不完全性脊髓损伤(ISCI)患者中的应用效果。方法选取2023年1月—2023年7月北京添福家中医康复医院收治的98例ISCI患者为研究对象,按随机数字表法将其分为对照组和观察组,每组49例。对照组行四肢联动训练,观察组在对照组的基础上加用黄芪桂枝五物汤治疗,持续治疗8周。对比两组的中医证候评分、肢体功能恢复情况、日常生活能力、生活质量。结果治疗前,两组肢体不遂、肢体麻木、大小便不调、伤处肿痛评分与简式Fugl-Meyer运动功能评分量表(FMA)、日常生活能力量表(ADL)及生活质量综合评定问卷(GQOLI-74)内各维度评分相比,组间差异无统计学意义(P>0.05);治疗8周后,观察组肢体不遂评分为(1.35±0.16)分,肢体麻木评分为(1.56±0.09)分,大小便不调评分为(1.23±0.07)分,伤处肿痛评分为(1.54±0.06)分,均低于对照组的(2.49±0.21)分、(2.15±0.13)分、(1.77±0.15)分、(2.35±0.11)分,FMA评分为(81.23±6.87)分,ADL评分为(79.84±5.01)分,均高于对照组的(70.59±5.20)分、(71.25±4.16)分,GQOLI-74内各个维度评分分别为(78.63±3.79)分、(77.35±4.16)分、(78.65±5.02)分、(79.86±3.95)分,均高于对照组的(70.59±3.13)分、(68.89±3.47)分、(66.79±4.11)分、(71.54±3.06)分,组间差异有统计学意义(P<0.05)。结论黄芪桂枝五物汤联合四肢联动训练可减轻ISCI患者的症状,促进肢体功能恢复,由此提高日常生活能力,改善生活质量。
Objective To analyze the effect of Huangqi Guizhi Wuwu Decoction combined with limb linkage training in patients with incomplete spinal cord injury(ISCI).Methods 98 ISCI patients admitted to Beijing Tianfujia Traditional Chinese Medicine Rehabilitation Hospital from January 2023 to July 2023 were selected as the research subjects,divided into a control group and an observation group using a random number table method,with 49 cases in each group.The control group received limb linkage training,while the observation group received Huangqi Guizhi Wuwu Decoction on the basis of the control group,and continued treatment for 8 weeks.Compare the Traditional Chinese Medicine syndrome scores,limb function recovery,daily living ability,and quality of life between two groups.Results Before treatment,comparison of scores for limb failure,numbness,irregular urination and defecation,and swelling and pain at the injury site between two groups and the scores of various dimensions in the Simplified Fugl-Meyer Motor Function Assessment Scale(FMA),Ability of Daily Living Scale(ADL),and Comprehensive General Quality of Life Inventory-74(GQOLI-74),there were no statistically significant differences between groups(P>0.05);8 weeks after treatment,the observation group's limb failure score was(1.35±0.16)points,limb numbness score was(1.56±0.09)points,urinary and fecal incontinence score was(1.23±0.07)points,and wound swelling and pain score was(1.54±0.06)points,which were lower than(2.49±0.21)points,(2.15±0.13)points,(1.77±0.15)points,and(2.35±0.11)points of the control group,the FMA score was(81.23±6.87)points,and the ADL score was(79.84±5.01)points,which were higher than(70.59±5.20)points and(71.25±4.16)points of the control group,the scores for each dimension within GQOLI-74 were(78.63±3.79)points,(77.35±4.16)points,(78.65±5.02)points,and(79.86±3.95)points,respectively,higher than(70.59±3.13)points,(68.89±3.47)points,(66.79±4.11)points,and(71.54±3.06)points were the control group,the differences between groups were statistically significant(P<0.05).Conclusion Huangqi Guizhi Wuwu Decoction combined with limb linkage training can alleviate symptoms in ISCI patients,promote limb function recovery,thereby improving daily living ability and quality of life.
