摘要
目的探求肾绞痛发作不同时期对行输尿管镜钬激光碎石术治疗输尿管中下段结石的疗效影响.方法回顾性收集2021年11月—2023年6月南京医科大学附属常州第二人民医院泌尿外科因输尿管中下段结石行输尿管镜钬激光碎石术的患者154例,分为两组,观察组115例,为肾绞痛首次发作14 d内接受手术治疗的患者;对照组39例,为肾绞痛发作超过14 d接受手术治疗的患者,比较两组的手术时间、输尿管狭窄发生概率、一次结石清除率、术前感染情况及拔管时间.结果对照组手术时间显著长于观察组(P<0.05),输尿管狭窄的发生率大于观察组(P<0.05),两组的一次结石清除率、术前感染情况及拔管时间均无明显差异(P>0.05).结论中下段输尿管结石的患者在肾绞痛发作后14 d内,接受输尿管镜激光碎石术可缩短手术时间,降低输尿管狭窄的发生概率,及时缓解患者痛苦.
Objective To investigate the effect of different stages of renal colic on the treatment of middle and lower ureteral calculi with ureteroscopic holmium laser lithotripsy.Methods Data of 154 patients with middle and lower ureteral calculi who underwent ureteroscopic holmium laser lithotripsy were retrospectively collected and patients were divided into two groups.115 cases in the observation group were treated with surgery within 14 days of the first attack of renal colic,and 39 cases in the control group were treated with surgery more than 14 days after renal colic attack.The operation time,probability of ureteral stricture,primary stone clearance rate,preoperative infection and extubation time were compared between the two groups.Results The operation time of the control group was significantly longer than that of the observation group(P<0.05),and the incidence of ureteral stricture was higher than that of the observation group(P<0.05).There were no significant differences in the stone clearance rate,preoperative infection and extubation time(P>0.05)between the two groups.Conclusion Ureteroscopic laser lithotripsy for patients with middle and lower ureteral calculi within 14 days after the onset of renal colic can shorten the operation time,reduce the incidence of ureteral stricture,and relieve the pain of patients in time.
