重症脑卒中患者早期加强营养支持的临床研究 被引量:2

摘要 目的探讨早期加强营养支持治疗重症脑卒中患者的临床疗效及对预后的影响.方法选择重症脑卒中患者48例,按随机数字表法分为治疗组24例和对照组24例.治疗组采用早期肠内外复合营养加强治疗,对照组按照传统方法进行营养支持治疗.比较两组患者治疗1周、2周时淋巴细胞计数、免疫球蛋白浓度、血红蛋白、血清白蛋白、转铁蛋白、前白蛋白、并发症发生率及预后的差异.结果治疗1周后,治疗组前白蛋白水平显著高于同期对照组,差异有统计学意义(P<0.05).治疗2周后,治疗组淋巴细胞计数、免疫球蛋白、白蛋白、前白蛋白、转铁血红蛋白均显著高于同期对照组,差异均有统计学意义(P<0.01或<0.05).治疗组治疗2周后格拉斯哥昏迷量表评分显著高于对照组,差异统计学意义(P<0.05).治疗组病死率、肺部感染发生率和褥疮防发生率明显低于对照组[4.2%(1/24)比16.7%(4/24),25.0%(6/24)比62.5%(15/24),4.2%(1/24)比12.5%(3/24)],差异有统计学意义(P<0.05).结论早期加强营养支持治疗能够改善重症脑卒中患者的营养状况,降低并发症发生率及降低病死率,改善患者预后.
