摘要
目的研究老年性白内障患者术前角膜后表面散光对总角膜散光的影响.方法横断面病例研究.收集老年性白内障患者97例97只眼,术前行pentacam检查,分析角膜后表面散光、总角膜散光、模拟角膜散光的分布及相关性,采用算术法和矢量法分析总角膜散光与模拟角膜散光的差异.结果角膜后表面散光的算术平均值为(-0.28±0.18)D,大于0.5 D的患眼占14.4%,其中88.7%为逆规散光.角膜前表面散光为顺规散光时,角膜前、后表面散光之间具有正相关性(r=0.445,P=0.007).模拟角膜散光与总角膜散光的矢量误差为(0.1±0.31)D@7°.角膜后表面散光与模拟角膜散光和总角膜散光之差之间具有正相关性(r=0.281,P=0.005).结论忽略角膜后表面散光会导致总角膜散光计算误差.对植入Toric人工晶状体的患者会引起顺规的欠矫及逆规的过矫.
Objective To describe the contribution of posterior corneal astigmatism to total corneal astigmatism in age-related cataract.Methods A rotating Scheimplfulg camera(Pentacam HR)was used to measure 97 eyes of 97 patients.The distribution of posterior corneal astigmatism,total corneal astigmatism and simulated keratometric astigmatism,and analyse the correlation between them.The error produced by estimating total corneal astigmatism using simulated keratometric astigmatism was calculated through vector and arithmetical analysis.Results The mean magnitude of posterior corneal astigmatism was(-0.28±0.18)D,and exceeded 0.5D in 14.4%.There was a strong correlation between the magnitude of anterior and posterior corneal astigmatism in eyes with with-the-rule anterior astigmatism(r=-0.228,p=0.024).The vector error between simulated keratometric astigmatism and total corneal astigmatis is(0.1±0.31)D@7°.The difference in magnitude between total corneal astigmatism and simulated keratometric astigmatism significantly increased with increasing the magnitude of posterior corneal astigmatism.Conclusion Neglecting posterior corneal astigmatism may lead to incorrect estimation of total corneal astigmatism.Selecting toric intraocular lenses could lead to overcorrection in eyes that have with-the-rule astigmatism and undercorrection in eyes that have against-the-rule astigmatism.
