一起通过呼吸道和接触传播引起的布鲁氏菌病疫情调查

摘要 目的针对2021年3月承德市某区一起布鲁氏菌病(简称布病)疫情,查明其起因及流行范围,为今后防控提供指导与依据.方法通过个案调查、医院就诊系统查询、重点人群筛查等方式开展病例搜索,对调查对象开展布鲁氏菌感染初筛及确证实验,对环境样本进行布鲁氏菌荧光定量PCR检测并对结果进行描述与分析.结果此次疫情共涉及8例确诊布病病例和1例隐性感染者;10份环境标本布鲁氏菌核酸检测均呈阳性,且均为羊种布氏菌;暴露于流产羊羔环境的聚餐人员感染率(9/12人)高于未暴露流产羊羔环境人员的感染率(0/6),差异有统计学意义(χ^(2)=6.25,P<0.05).结论暴露于流产羊羔环境是造成此次布病感染的危险因素,呼吸道和皮肤黏膜接触传播是主要传播途径. Objective In order to find out the cause of brucellosis,clarify the epidemic scope,and provide a scientific basis for the prevention of similar outbreaks in the future.Methods Case search was carried out through case investigation,hospital treatment system inquiry,key population screening and other means.Brucella infection preliminary screening and confirmation experiments were carried out on the investigation subjects,and brucella fluorescence quantitative PCR detection was carried out on environmental samples.The results were described and analyzed.Results A total of 8 confirmed cases of brucellosis and 1 recessive infected case have been reported.All 10 environmental samples were positive for brucella,and all of them were Brucella sheep.The infection rate of those exposed to the environment after blowing lambs(9/12)was higher than that of those who left before blowing lambs(0/6),and the difference was statistically significant(χ^(2)=6.25,P<0.05).Conclusion Exposure to the environment after blowing lambs is a risk factor for brucellosis infection,and respiratory transmission is one of the transmission routes in this outbreak.
