急性期未服药重性抑郁障碍患者全脑弥散张量改变的临床研究 被引量:1

Alterations in whole brain diffusion tensor imaging in unmedicated patients with acute major depressive disorder

摘要 目的:探讨未服药急性期重性抑郁障碍(MDD)患者在弥散张量成像(DTI)成像下的全脑白质微结构损伤的特征表现及其与抑郁严重程度的关系。方法:将2017年10月至2018年10月在广州市第一人民医院门诊就诊或者病房住院的29例MDD患者纳入MDD组,同期招募性别、年龄、受教育程度及BMI匹配的25例健康人作为对照组。利用DTI技术进行全脑结构各向异性分数(FA)检测,采用汉密顿抑郁量表(HAMD)进行抑郁评定,对出现脑白质FA值下降的脑区与抑郁量表评分进行相关性分析。结果:MDD组治疗前左侧上枕额束、右侧上枕额束、内囊后肢(左侧0.75±0.02比0.73±0.03,右侧0.74±0.02比0.73±0.03)及穹隆等部位的FA值均低于对照组(P<0.05);MDD双侧内囊后肢、上枕额束、穹隆的FA值与抑郁症状严重程度之间未见明显相关性(P>0.05)。结论:双侧上枕额束、双侧内囊后肢及穹隆的白质纤维束完整性的改变可能是MDD病理机制中的关键因素。 Objective:To investigate the characteristics of whole brain white matter(WM)microstructure damage as displayed by diffusion tensor imaging(DTI)and its correlation with severity of depression in unmedicated patients with acute major depressive disorder(MDD).Methods:Twenty-nine MDD patients who visited our outpatient clinics or hospitalized in Guangzhou First People’s Hospital between October 2017 and October 2018 were enrolled as the MDD group.A contemporary cohort of 25 healthy subjects matched for gender,age,education level and BMI were recruited as the control group.DTI was used for assessment of whole-brain structural anisotropy score(FA)of all subjects.HAMD scale was used to evaluate the severity of depression.Correlation between the brain regions with decreased white matter FA values and the Hamilton depression scale(HAMD)score was analyzed.Results:The FA values of left and right superior fronto-occipital fasciculus(SFO),posterior limbs of internal capsule(left 0.75±0.02 vs 0.73±0.03,right 0.74±0.02 vs 0.73±0.03)and fornix in unmedicated MDD patients were significantly lowered than those in healthy controls(P<0.05).No correlation was found between the lowered FA values of bilateral SFO fasciculus,posterior limbs of internal capsule and severity of depression(all P>0.05).Conclusion:The altered integrity of white matter tracts in bilateral SFO fasciculus,posterior limbs of internal capsule and fornix play a critical role in the pathogenesis of MDD.
