摘要
目的:分析高分辨磁共振(MRI)在肛内巨大有柄型尖锐湿疣中的应用,探讨MRI在肛内巨大有柄型尖锐湿疣中的诊断价值。方法:回顾性分析6例肛内巨大有柄型尖锐湿疣患者MRI影像资料,包括高分辨磁共振T1WI、T2WI、T2WI-FS、DWI及增强检查资料,以手术结果为金标准,以HPV阳性指标为重要参考依据,分析肛内巨大有柄型尖锐湿疣的MRI影像特点。结果:本组6例肛内巨大有柄型尖锐湿疣患者中,病变长径为3.7~5.4 cm,呈菜花状4例、鸡冠状1例、蕈伞状1例。位置在肛白线外4例,肛白线与齿状线间1例,累及齿状线1例。浸润深度均不超过皮肤深层。病变在T1WI上呈稍低信号,T2WI及T2WI压脂像上呈稍高信号且信号多不均匀,DWI受限呈高信号,ADC图呈稍高信号,增强呈中等度持续性强化,病变边缘可见“帽征”。所有患者高分辨MRI检查所见与手术结果一致,诊断符合率100%。结论:高分辨MRI成像是肛内巨大有柄型尖锐湿疣影像检查的重要方法,能清晰显示病变范围及特点,为临床诊断和治疗提供重要依据。
Objective To explore the diagnostic value of high-resolution MRI in inspecting intraanal giant moist wart(MW)with handle.Methods Reviewed the image data of 6 cases with intraanal MW with handle treated in author’s hospital(2018-11-2022-03),including high-resolution MRI T1WI,T2WI,T2WI-FS,DWI,and enhanced inspection materials,taking operative results as gold-standard,and HPV positive norms as important reference basis,analysed the MW’s MRI characteristics.Results In the 6 cases,lesion-length were(3.7-5.4)cm:cauliflower-like,4 cases;cockcomb-like,1;gilfungns-like,1;outside anal white line,4;between anal white line and dentate line,1;involved dentate line,1;but their infiltration depth were all within skin depth.These lesions showed little lower signal on T1WI,but,on T2WI and T2WI autogram image,little higher signals,and no-smooth;DWI limited,high signal;ADC figure,little higher,enhanced to mid-,and continously intensive one,and"cap-feature"showing at lesion-edge.High-resolution MRI of patients all were of accordance with operative results,thus accordante rate was up to 100%.Conclusion High-resolution MRI is an important imaging inspection method in diagnosis of intraanal MW with handle,can clearly show lesion range and feature,providing important clinical diagnostic&treatment basis.
