摘要
目的:调查糖尿病患者低血糖恐惧感现状,并分析其影响因素,为糖尿病患者低血糖恐惧感的改善提出可参考建议。方法:采用便利抽样法选择2018年6—11月期间在洛阳市内4所医院的内分泌科的196例住院糖尿病患者作为调研对象。对研究对象实施问卷调查,运用SPSS21.0统计软件进行数据分析。结果:本研究中196例糖尿病患者低血糖恐惧感得分为0~49分,中位数15,四分位数(12.25,20.00),患者的低血糖恐惧感属于较高的水平。单因素分析显示,不同性别、婚姻状况、糖尿病类型、病程、低血糖频次、低血糖严重程度、最近一次糖化血红蛋白指数的糖尿病患者的低血糖恐惧感评分差异具有统计学意义(P<0.05)。多元线性回归分析显示,低血糖恐惧得分能被性别、婚姻状况、病程、糖尿病类型、低血糖频次、最近一次糖化血红蛋白解释的百分比为30.3%。结论:大多数糖尿病患者对低血糖有恐惧感。在临床护理工作中,医护人员对女性、离婚或丧偶、病程大于20年、糖尿病类型为1型、低血糖发生频次多于1次/周、最近一次糖化血红蛋白≥7%的患者应做好低血糖相关知识的指导来缓解患者的低血糖恐惧感。
Objective:To investigate the status quo of hypoglycemic fear in diabetic patients and analyze its influencing factors,so as to provide referencesfor the improvement of hypoglycemic fear.Methods:Totally 196 diabetic patients admitted to the Department of Endocrinology of four hospitals in Luoyang city from June to November in 2018 were selected as subjects according to the inclusive criteria.Questionnaires were used to gather relevant information and SPSS 21.0,a statistical software was used for data analysis.Results:In this study,196 patients with diabetes had a score range of 0 to 49,median 15,and interquartile interval(12.25,20.00),indicating a higher level of hypoglycemic fear.Single factor analysis showed that different gender,marital status,type of diabetes mellitus severity,disease duration,low frequency,low blood sugar,the latest glycosylated hemoglobin index of diabetic hypoglycemia fear score differences statistically significant(P<0.05),multiple linear regression analysis showed that the total hypoglycemic fear score from mean sum of squared residuals can be gender,marital status,duration,type of diabetes,low frequency,severity,a recent glycosylated hemoglobin explain percentage is 30.3%.Conclusion:Most diabetic patients have a fear of hypoglycemia.In clinical nursing,medical staff should provide guidance on related knowledge of hypoglycemia for patients with female,divorce or widowhood,disease duration more than 20 years,type 1 diabetes,occurrence frequency of hypoglycemia more than once a week,and latest glycosylated hemoglobin≥7%,so as to alleviate patients’fear of hypoglycemia.
