摘要
目的探讨围术期个性化心理护理对肺癌患者术前焦虑及促进术后恢复的作用。方法将我院2018年4月~2019年4月收治的围手术期肺癌患者84例随机分组,观察组施以个性心理护理(n=42),对照组施以常规护理(n=42),对比两组患者入科时及术前焦虑、抑郁程度,皮质醇水平,术后恢复指标,健康状况。结果两组入科时焦虑、抑郁程度无差异(P>0.05),干预后,焦虑、抑郁程度均下降,且观察组下降程度更为明显(P<0.05)。两组入科时血清皮质醇水平无差异(P>0.05),干预后,两组血清皮质醇水平均明显下降,且观察组下降程度更为明显(P<0.05)。观察组术后离床活动时间、实施心电监控时间、引流时间经观察均少于对照组(P<0.05)。两组干预前,健康状况量表躯体、精力、睡眠、情感各维度评分经测验无差异(P>0.05),干预后各测验值均有降低,且观察组改善更为显著(P<0.05)。结论针对肺癌手术患者在围术期重视施以个性化心理护理,可减轻术前焦虑、抑郁,缩短病程,利于康复。
Objective To explore the effect of personality psychological nursing during perioperative period on relieving preoperative anxiety and promoting the recovery process of patients with lung cancer.Methods 84 lung cancer patients underwent operation were selected from April 2017 to April 2019 in our hospital.The patients were divided into two groups by random number table extraction method.They were given personality psychological nursing(observation group,n=42)and routine nursing(control group,n=42)during the perioperative period,The level of anxiety and depression,the cortisol level,rehabilitation indicators and health status scores before and after operation were compared between two groups.Results Before intervention,there was no difference between the two groups in the degree of anxiety and depression(P>0.05),after intervention,the degree of anxiety and depression all decreased,and the decrease in the observation group was more obviously(P<0.05).There was no difference in serum cortisol level between the two groups before intervention(P>0.05).After intervention,the tserum cortisol level all decreased significantly,and the decrease in the observation group was more obviously(P<0.05).The time of leaving bed,ECG monitoring and drainage in the observation group were less than those in the control group(P<0.05).Before the intervention,there was no difference in the scores of body,energy,sleep and emotion dimensions between the two groups(P>0.05).After the intervention,the scores of body,energy,sleep and emotion dimensions in the observation group was better than those in the control group(P<0.05).Conclusion The personality psychological nursing during perioperative period for patients undergoing lung cancer surgery can alleviate preoperative anxiety and depression,improve emotional status,shorten the course of disease and facilitate the recovery of health status.
