2006—2017年铜绿假单胞菌的分布及耐药性变迁 被引量:3

The Distribution and Antibiotic Resistance of Pseudomonas aeruginosa in the First Affiliated Hospital of Guangxi Medical University From 2006 to 2017

摘要 目的了解我院2006年至2017年临床分离的铜绿假单胞菌细菌分布和耐药性变迁,为临床合理使用抗菌药物提供参考。方法回顾性分析2006年1月至2017年12月在广西医科大学第一附属医院就诊的门急诊患者和住院患者中分离的铜绿假单胞菌,对其细菌分布及其药物敏感性进行分析。结果共分离出铜绿假单胞菌10004株,97.72%菌株分离自住院患者,53.25%菌株分离自痰标本。铜绿假单胞菌对抗菌药物的耐药率总体变化不大,平均耐药率低于30%。铜绿假单胞菌对哌拉西林/他唑巴坦和阿米卡星的耐药率最低。铜绿假单胞菌对亚胺培和美罗培南的耐药率高于20%,高于头孢他啶和头孢吡肟的耐药率。ICU科分离的铜绿假单胞菌耐药率最高,其次是外科和内科,耐药率最低的是儿科。结论我院分离的铜绿假单胞菌耐药情况严重;分离自不同年龄段、科室和标本类型对抗菌药物的耐药性相关较大。医院应继续开展病原菌分布及其耐药性监测,用于指导临床医生合理使用抗生素,控制耐药菌株在医院中的流行。 Objective To investigate the distribution and antibiotic resistance of Pseudomonas aeruginosa in the first affiliated hospital of Guangxi medical university from 2006 to 2017,so as to provide reference for rational use of antibiotics.Methods Retrospective analysis of the distribution and antibiotic resistance of Pseudomonas aeruginosa from January 2006 to December 2017 in the first affiliated hospital of Guangxi medical university.Results A total of 10004 isolates of Pseudomonas aeruginosa were collected in the first affiliated hospital of Guangxi medical university.Majority of 10004 isolates of Pseudomonas aeruginosa were from inpatients(97.72%)and sputum samples(53.25%).The average resistance rates of Pseudomonas aeruginosa to antibiotics were less than 30%.The Pseudomonas aeruginosa isolates showed the lowest resistance rate to piperacillin/tazobactam and amikacin.The resistance rate of Pseudomonas aeruginosa to imipenem and meropenem was more than 20%,which was higher than that of ceftazidime and cefepime.The resistance rates of Pseudomonas aeruginosa isolated from ICU were the highest,followed by surgery and internal medicine,while the lowest resistance rates of Pseudomonas aeruginosa were from pediatrics.Conclusions The resistance rates of Pseudomonas aeruginosa collected from the first affiliated hospital of Guangxi medical university are serious.The resistance rates of Pseudomonas aeruginosa vary with patient age,specimen type and the department where the strain was isolated.Surveillance of antimicrobial resistance is important for rational antimicrobial therapy and effective prevention and control of the spread of resistant Pseudomonas aeruginosa strains.
