摘要
目的:探讨沙库巴曲缬沙坦钠对射血分数偏低慢性心力衰竭患者心功能及预后的影响。方法:选取2018年5月至2019年5月深圳市罗湖区人民医院收治的90例射血分数偏低(<40%)慢性心力衰竭患者作为研究对象,随机分为对照组与观察组,每组各45例。常规抗心力衰竭药物治疗的同时,分别给予地高辛、地高辛加沙库巴曲缬沙坦钠片治疗。测定治疗前及治疗2个月两组心脏功能[舒张期左心室后壁厚度(LVPWD)、左心室射血分数(LVEF)及左室舒张末期内径(LVEDD)],N端前体脑钠肽(NTProBNP)、血清肌钙蛋白I(cTnI)及可溶性生长刺激表达基因2蛋白(sST2)水平;比较疗效并统计用药期间预后不良率。结果:与治疗前相比,两组治疗后LVEF均有所增高,LVPWD、LVEDD、NTProBNP、cTnI及sST2均有所下降,其中观察组LVEF高于对照组,LVPWD、LVEDD、NTProBNP、cTnI及sST2低于对照组(均P<0.05)。观察组有效率为93.33%,高于对照组的77.78%(P<0.05),观察组用药期间预后不良率为2.22%,低于对照组的17.78%(P<0.05)。结论:沙库巴曲缬沙坦钠可有效改善射血分数偏低慢性心力衰竭患者的心功能,改善预后。
Objective:To investigate the effect of Sacubitril valsartan sodium(Entresto)on cardiac function and prognosis of patients with chronic heart failure and reduced ejection fraction.Methods:Included in this study were 90 patients with chronic heart failure and reduced ejection fraction(<40%)admitted to Luohu District People’s Hospital in Shenzhen between May 2018 and May 2019.The subjects were randomly divided into control group and study group(n=45 each).In addition to treatment with conventional drugs against heart failure,the two groups were treated with digoxin and digoxin combined with Entresto,respectively.Cardiac function[left ventricular posterior wall thickness at end-diastole(LVPWD),left ventricular ejection fraction(LVEF)and left ventricular end-diastolic diameter(LVEDD)],and the levels of N-terminal pro-brain-type natriuretic peptide(NTProBNP),serum troponin I(cTnI)and soluble growth stimulation expressed gene 2(sST2)were detected before treatment and after 2 months of treatment.The treatment efficacy was compared between groups.Rate of poor prognosis during medication was recorded.Results:After treatment,LVEF in both groups increased while LVPWD,LVEDD,NTProBNP,cTnI and sST2 decreased,compared with baseline.The LVEF was higher,while LVEPWD,LVEDD,NTProBNP,cTnI and sST2 were lower in the study group than those in the control group(P<0.05).The rate of treatment response was higher in the study group was higher than that in the control group(93.33%vs 77.78%)(P<0.05).The poor prognosis rate in the study group was lower than that in the control group(2.22%vs 17.78%)(P<0.05).Conclusion:Entresto may effectively improve the cardiac function and prognosis of patients with chronic heart failure and reduced ejection fraction.
