摘要
目的:探讨外周血中性粒细胞淋巴细胞比值(neutrophil-lymphocyte ratio,NLR)、血小板淋巴细胞比值(plateletlymphocyte ratio,PLR)与慢性阻塞性肺病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者肺部细菌感染的关系。方法:选取2019年11月至2020年2月于我院全科医学科住院的113例AECOPD患者,按照痰细菌培养结果分为痰细菌培养阴性组A组(64例)及痰细菌培养阳性组B组(49例),收集两组患者中性粒细胞绝对值、淋巴细胞绝对值、血小板计数,计算NLP、PLR值,比较两组患者NLR、PLR的变化,Pearson检验分析NLR、PLR与C反应蛋白(CRP)、降钙素原(PCT)的相关性。结果:A组患者NLR水平低于B组(P<0.05);A组患者PLR水平与B组比较差异无统计学意义(P>0.05)。B组患者NLR水平与PLR、CPR、PCT、白细胞计数、住院天数呈正相关(r分别为0.725、0.442、0.348、0.475、0.211)。结论:NLR可能是AECOPD患者肺部细菌感染潜在炎症标志物。
Objective:To explore the relationship between peripheral blood neutrophil lymphocyte ratio(NLR),platelet lymphocyte ratio(PLR)and bacterial infection of lung in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:113 patients with AECOPD who were hospitalized in the Department of General Medicine of our hospital from November 2019 to February 2020 were selected and divided into 2 groups according to the sputum bacterial culture results,that is,sputum bacterial culture negative group A group(64 cases)and sputum bacteria culture positive group B(49 cases),collected the absolute values of neutrophils,lymphocytes,and platelet counts of the two groups of patients and calculated NLP and PLR values,then compared the changes of NLR and PLR of the two groups of patients,Pearson analysis the correlation between NLR,PLR and C-reactive protein(CRP),procalcitonin(PCT).Results:The NLR level in group A(6.60±5.11)was lower than that in group B(9.72±6.84)(P=0.021);the PLR level in group A(200.24±141.64)was not significantly different from group B(196.08±111.18)(P=0.866).Pearson correlation analysis showed that the level of NLR in group B was positively correlated with PLR,CPR,PCT,white blood cell count,and length of hospital stay(r was 0.725,0.442,0.348,0.475,0.211,respectively).Conclusion:NLR may be a potential inflammatory marker of bacterial infection in the lungs of AECOPD patients.
