摘要
目的分析血管内支架植入辅助弹簧圈栓塞治疗颅内动脉瘤的疗效和安全性。方法选取2017年8月至2019年8月济源市人民医院收治的76例颅内动脉瘤患者为研究对象,按照随机数字表法分为内支架组与夹闭组,每组38例。夹闭组进行常规开颅夹闭术治疗,内支架组进行血管内支架植入辅助弹簧圈栓塞治疗。对比两组疗效、神经功能缺损评分量表(NIHSS)评分、血清指标水平及并发症情况。结果内支架组Rankinscale评分优于夹闭组(P<0.05)。术后1周,两组NIHSS评分均低于术前,且内支架组低于夹闭组(P<0.05)。术后1周,内支架组SI00β蛋白、可溶性细胞间黏附分子-1(s ICAM-1)水平低于夹闭组(P<0.05)。内支架组并发症发生率[5.26%(2/38)]较夹闭组[21.05%(8/38)]低(P<0.05)。结论颅内动脉瘤患者采用血管内支架植入辅助弹簧圈栓塞术效果确切,可促进神经功能和机体恢复,且并发症发生率低,具有较高安全性。
Objective To analyze the efficacy and safety of endovascular stent implantation assisted coil embolization for intracranial aneurysms.Methods Seventy-six patients with intracranial aneurysm in Jiyuan Peoples’Hospital from August 2017 to August 2019 were selected.They were randomly divided into inner stent group and clipping group,with 38 cases in each group.The clipping group was treated with routine craniotomy,and the stent group was treated with endovascular stent implantation assisted by coil embolization.The curative effect,NIHSS score,serum level and complications of the two groups were compared.Results The RS score of the stent group was better than that of the clipping group(P<0.05).One week after surgery,the neurological deficit score(NIHSS)in both groups was lower than that before surgery,and that in the stent group was lower than that in the clipping group(P<0.05).One week after operation,the levels of SI00βprotein and soluble intercellular adhesion molecule-1(sic AM-1)in inner stent group were lower than those in clipping group(P<0.05).The complication rate of stent group was 5.26%(2/38),which was lower than that of clipping group[21.05%(8/38),P<0.05].Conclusions Endovascular stent implantation assisted by coil embolization is effective on patients with intracranial aneurysm,which can promote nerve function and body recovery,and the complication rate is low,with high security.
