摘要
目的研究磁共振成像(MRI)相关影像学参数在伴腕关节病变类风湿骨性病变分期诊断中的应用价值。方法选取2018年3月至2021年3月河南省直第三人民医院收治的98例伴腕关节病变类风湿骨性病变患者为研究对象,均行MRI检查,统计相关影像学参数[早期强化率(EER)、最大强化率(MER)、平均强化率(AER)、达峰时间(TTP)]及OMERACT-RAMRIS定量评分。比较活动期、稳定期患者MRI检查的影像学参数、定量评分,分析影像学参数与定量评分的相关性及影像学参数对活动期患者的诊断价值。结果伴腕关节病变类风湿骨性病变患者活动期EER、AER、MER大于稳定期,TTP小于稳定期(P<0.05);活动期定量评分高于稳定期(P<0.05);经Pearson相关性分析,EER、AER、MER与定量评分呈正相关,TTP与定量评分呈负相关(P<0.05);经受试者工作特征曲线分析,EER、AER、MER、TTP诊断活动期患者曲线下面积值分别为0.714、0.689、0.754、0.729。结论MRI检查能清晰显示腕关节病变类风湿骨性病变患者病理性改变,其相关影像学参数及定量评分能准确客观评价活动程度,且二者具有一定相关性,诊断价值较高。
Objective To study the value of related imaging parameters of magnetic resonance imaging(MRI)in staging diagnosis of rheumatoid bone disease with wrist joint disease.Methods Ninety-eighty patients with rheumatoid bone disease with carpal joint disease in the Third People’s Hospital Directly Under Henan Province from March 2018 to March 2021 were selected for MRI examination,and the relevant imaging parameters[early enhancement rate(EER),maximum enhancement rate(MER),average enhancement rate(AER),time to peak(TTP)]and omeact-ramris quantitative score were counted.The imaging parameters and quantitative scores of MRI in patients with active and stable phase were compared,and the correlation between imaging parameters and quantitative scores and the diagnostic value of imaging parameters in patients with active phase were analyzed.Results EER,AER and MER in active phase were higher than those in stable phase,and TTP was lower than that in stable phase(P<0.05).The quantitative score in active phase was higher than that in stable phase(P<0.05).Pearson correlation analysis showed that EER,AER,MER were positively correlated with quantitative scores,while TTP was negatively correlated with quantitative scores(P<0.05).According to ROC curve analysis,the AUC values of patients with active diagnosis of EER,AER,MER and TTP were 0.714,0.689,0.754 and 0.729 respectively.Conclusions MRI examination can clearly show the pathological changes of patients with rheumatoid bone disease of wrist joint disease.Its related imaging parameters and quantitative scores can accurately and objectively evaluate the degree of activity,and the two have a certain correlation,with high diagnostic value.
