摘要
目的探讨桂枝活血汤联合中医正骨手法在老年桡骨远端骨折患者中的应用效果。方法选取2018年7月—2021年4月我院收治的36例老年桡骨远端骨折患者为研究对象,按随机数字表法分为两组,各18例。对照组采用中医正骨手法治疗,试验组在对照组基础上加用桂枝活血汤治疗,随访至治疗后3个月。对比两组中医症候积分、疼痛视觉模拟评分(VAS)及腕关节功能。结果治疗前,两组的中医症候积分、VAS评分比较,组间差异无统计学意义(P>0.05);治疗后3个月,试验组的肿胀、瘀斑、肢体萎缩症状及VAS评分分别为(1.21±0.18)分、(1.09±0.20)分、(1.21±0.35)分、(1.42±0.35)分,均低于对照组的(1.74±0.20)分、(1.57±0.26)分、(1.86±0.41)分、(2.30±0.57)分,组间差异有统计学意义(P<0.05)。治疗前,两组的腕关节各方向活动度的比较,组间差异无统计学意义(P>0.05);治疗后3个月,试验组的腕关节背伸度、掌屈度、桡偏度活动度分别为(40.71±2.89)°、(50.11±2.37)°、(13.75±1.42)°,均大于对照组的(36.90±1.48)°、(45.29±2.53)°、(10.46±1.28)°,组间差异有统计学意义(P<0.05)。结论桂枝活血汤联合中医正骨手法治疗老年桡骨远端骨折的效果确切,可有效减轻患者关节疼痛肿胀程度,改善腕关节功能。
Objective To explore the effect of Guizhi Huoxue Decoction combined with traditional Chinese medicine bone-setting manipulation in elderly patients with distal radius fractures.Methods A total of 36 elderly patients with distal radius fractures admitted to our hospital from July 2018 to April 2021 were selected as the research objects,and divided into two groups according to the random number table method,with 18 cases in each group.The control group was treated with traditional Chinese medicine bone-setting manipulation,and the experimental group was treated with Guizhi Huoxue Decoction on the basis of the control group.The patients were followed up for 3 months after treatment.The TCM symptom score,pain visual analog scale(VAS)score and wrist function were compared between the two groups.Results Before treatment,there were no significant differences in TCM symptom scores and VAS scores between the two groups(P>0.05);3 months after treatment,the swelling,petechiae,limb atrophy symptoms and VAS scores in the experimental group were(1.21±0.18)points,(1.09±0.20)points,(1.21±0.35)points,and(1.42±0.35)points,respectively,which were lower than those in the control group(1.74±0.20)points,(1.57±0.26)points,(1.86±0.41)points,and(2.30±0.57)points,and the differences between the groups were statistically significant(P<0.05).Before treatment,there were no significant differences in the range of motion of the wrist joint between the two groups in all directions(P>0.05);3 months after treatment,the wrist dorsiflexion,palmar flexion,and radial deviation mobility of the experimental group were(40.71±2.89)°,(50.11±2.37)°,and(13.75±1.42)°,respectively,which were greater than those of the control group(36.90±1.48)°,(45.29±2.53)°,and(10.46±1.28)°,and the differences between the groups were statistically significant(P<0.05).Conclusion Guizhi Huoxue Decoction combined with traditional Chinese medicine bone-setting manipulation is effective in treating elderly distal radius fractures,which can effectively reduce the pain and swelling of patients’joints and improve wrist joint function.
