摘要
目的:观察99Tc-亚甲基二膦酸盐(商品名:云克)联合复方倍他米松治疗血友病性滑膜炎的临床疗效。方法:选取我科2017年3月至2018年9月收治的15例血友病性滑膜炎患者,进行云克联合复方倍他米松治疗,观察且对比治疗前后关节出血次数、视觉模拟评分法(VAS)评分、血友病关节健康评估表(HJHS)评分,以及治疗过程中药物不良反应。结果:治疗后患者关节出血次数减少,明显有效6例,有效8例,总有效率为93.3%,治疗前后差异有统计学意义(P<0.05)。治疗后3 d,VAS评分较治疗前均有所降低,差异有统计学意义(P<0.05)。治疗6月后HJHS评估1人(6.7%)靶关节较前有所改善,其余患者(93.3%)无明显进展,没有患者的靶关节损害程度较前进展。患者治疗期间均未发生明显药物不良反应。结论:云克联合复方倍他米松关节腔内注射治疗血友病性滑膜炎疗效显著,可以明显减少出血次数,缓解疼痛程度,延缓血友病患者靶关节的损伤进展,且安全性较高。
Objective:To observe the clinical efficacy of technetium-99 methylene diphosphate(99 Tc-MDP)combined with compound betamethasone in the treatment of hemophilic synovitis.Methods:Fifteen patients with hemophilic synovitis who were admitted to our hospital from Mar 2017 to Sep 2018 were treated with99 Tc-MDP combined with compound betamethasone.The number of joint bleeding,visual analogue scale(VAS),and Hemophilia Joint Health Assessment Form(HJHS)were analyzed before and after treatment.Adverse drug reactions during treatment were observed.Results:After treatment,the number of joint bleeding was reduced.which was markedly effective in 6 cases and effective in 8 cases.The total effective rate was 93.3%.The difference was statistically significant(P<0.05).Three days after treatment,the VAS score was lower,and the difference was statistically significant(P<0.05).After six months of treatment,one case(6.7%)of the HJHS had improved target joints,and the remaining patients(93.3%)had no significant progress.No patients had more progress in target joint damage.No significant adverse drug reactions occurred during the treatment of the patients.Conclusion:Intra-articular injection of99 Tc-MDP combined with compound betamethasone is effective in the treatment of hemophilic synovitis,which can significantly reduce the number of bleeding,relieve pain,delay the progression of target joint damage,and has higher safety.
