孕期镍砷暴露与新生儿脐带血氧化应激指标相关性

Correlation between nickel-arsenic exposure during pregnancy and oxidative stress in neonatal umbilical cord blood

摘要 目的探讨孕中期、孕晚期、分娩时孕妇镍(Ni)、砷(As)暴露与新生儿氧化应激指标关系及其密切程度。方法于2013—2016年选取甲地(有矿开采)和乙地(无矿开采)孕中期(71对)、孕晚期(171对)、分娩时(206对)孕妇和新生儿作为研究对象。收集孕妇基本资料并在孕中期、孕晚期、分娩时采集孕妇静脉血和新生儿脐带血。以电感耦合等离子质谱法(ICP-MS)测定孕妇静脉血Ni、As水平,酶联免疫吸附法(ELISA)检测新生儿脐带血氧化应激指标8-异构前列腺素2α(8-epi-PGF2α)、3-硝基酪氨酸(3-NT)及8-羟基脱氧鸟苷(8-OHdG)含量。多重线性回归分析孕期孕妇静脉血Ni、As暴露水平与新生儿脐带血氧化应激指标关系。结果除孕晚期Ni外,孕中期、分娩时甲地孕妇静脉血Ni、As水平显著高于乙地(P<0.05);除孕中、晚期8-epi-PGF2α外,孕中期、孕晚期、分娩时甲地新生儿脐带血3-NT、8-OHdG、8-epi-PGF2α高于乙地(P<0.05)。多重线性回归分析显示,孕中期孕妇静脉血Ni[β:0.110(95%CI:0.019~0.202)]、As[β:0.290(95%CI:0.115~0.466)]与3-NT呈正相关;分娩时孕妇静脉血Ni与3-NT无相关性[β:0.044(95%CI:-0.004~0.092)];孕中期、孕晚期、分娩时孕妇静脉血Ni、As与8-epi-PGF2α、8-OHdG均无相关性(P>0.05)。结论孕期镍、砷暴露与新生儿脐带血氧化应激指标有一定关联,可能导致氧化应激,孕中期金属暴露更适合作为胎儿内暴露生物标志。 Objective To explore the relationship between nickel(Ni),arsenic(As)exposure and oxidative stress indexes in neonates during second trimester,third trimester and delivery and its degree.Methods From 2013 to 2016,71 pairs in the second trimester,171 pairs in the third trimester and 206 pairs at delivery of pregnant women and newborns were selected as study subjects in A area with mining and B area without mining.The basic data of pregnant women were collected,maternal venous blood were collected in second trimester,third trimester and delivery for the determination of Ni and As by ICP-MS,neonatal umbilical cord blood were collected for determination of oxidative stress indexes,such as 8-epi prostaglandinF2α(8-epi-PGF2α),3-nitro-tyrosine(3-NT)and 8-hydroxydeoxyguan-osine(8-OHdG)by ELISA.The relationship between Ni and As levels in venous blood of pregnant women and oxidative stress indexes in neonates was analyzed by multiple linear regression.Results The levels of Ni and As in maternal venous blood during three periods except Ni in second trimester in A area were significantly higher than B area(P<0.05);The levels of 3-NT,8-epi-PGF2αand 8-OHdG of newborns during three periods except 8-epi-PGF2αin the second and third trimester in A area were higher than B area(P<0.05).Multiple linear regression analysis showed positive correlation between Ni[β:0.110(95%CI:0.019-0.202)],As[β:0.290(95%CI:0.115-0.466)]in maternal venous blood in the second trimester and 3-NT of newborns;There was no correlation between Ni in maternal venous blood in delivery and 3-NT of newborns[β:0.044(95%CI:-0.004-0.092)],as well as between Ni,As in maternal venous blood and 8-epi-PGF2α,8-OHdG of newborns in three periods(P>0.05).Conclusion Ni,As exposure during pregnancy may cause oxidative stress injury to newborns,the second trimester is the more suitable period for collection of biomarker of intrauterine exposure.
