摘要
目的探讨遮眼预处理对降低眼科学龄前期患儿全麻术后急性谵妄发生率及减轻谵妄程度的效果。方法选取2019年8月—2020年7月在浙江省某三级甲等医院择期行眼科全麻手术的学龄前期患儿为研究对象,采用随机数字表法分为试验组和对照组各150例。试验组患儿在卡通动画视频宣教基础上结合遮眼预处理;对照组患儿在术前进行卡通动画视频常规宣教。比较两组术前焦虑评分、术后急性谵妄发生率、谵妄程度评分和术后护理照护难度评分。结果本研究最终完成279例,试验组133例、对照组146例。试验组术前焦虑评分、术后急性谵妄发生率及谵妄程度评分、术后护理照护难度评分均显著低于对照组,差异有统计学意义(P<0.001)。结论遮眼预处理能降低眼科学龄前期患儿术前焦虑水平和术后急性谵妄发生率,减轻谵妄程度,并降低护理照护难度,有效提高了眼科学龄前期患儿的围手术期安全水平。
Objective To explore the effect of pretreatment with eye covering on reducing the incidence of acute delirium and the score of delirium after ophthalmic surgery with general anesthesia in pre-school aged children.Methods A total of 300 pre-school aged children who underwent general anesthesia for selective ophthalmic surgery in a tertiary class A hospital from August 2019 to July 2020 were selected and divided into experimental group and control group with 150 cases in each group by random number table.Children in the control group received routine education by cartoon animation videos before surgery.Children in the experimental group received eye-covering pretreatment on the basis of cartoon animation videos.The differences in preoperative anxiety score,postoperative acute delirium incidence,score of delirium degree,and score of postoperative nursing care difficulty were compared between the two groups.Results 279 cases were completed in this study,including 133 cases in the experimental group and 146 cases in the control group.The score of preoperative anxiety,the incidence of acute delirium and score of delirium degree,the score of postoperative nursing care difficulty in the experimental group were significantly lower than those of the control group,and the differences were statistically significant(P<0.001).Conclusion Eye-covering pretreatment can reduce the preoperative anxiety level,the incidence and degree of postoperative acute delirium,and the difficulty of nursing care,and effectively improve the perioperative safety of ophthalmological pre-school aged children.
