摘要
目的:2型糖尿病(T2DM)患者血清脂蛋白相关磷脂酶A2(Lp-PLA2)水平与血管病变发生的相关性。方法:回顾性分析2015年9月~2017年9月在本院的235例被确诊的T2DM患者,根据其有无血管并发症分为无血管病变组127例和并发血管病变组108例,收集同期进行糖尿病筛查的健康人群相关资料作为正常对照组100例。测定各组患者空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、白介素(IL-6)、C反应蛋白(CRP)和Lp-PLA2水平,采用Pearson相关性分析Lp-PLA2与血管病变并发症各指标之间的相关性,ROC曲线分析其诊断价值。结果:与无血管病变组相比,并发血管病变组FPG、2hPG、HbA1c、TC、TG、LDL-C、IL-6、CRP和Lp-PLA2水平显著升高,HDL-C水平显著下降;Pearson相关性分析显示Lp-PLA2与HbA1c、IL-6和CRP呈显著正相关关系,ROC曲线分析显示Lp-PLA2对T2DM并发血管病变诊断AUC为0.873。结论:Lp-PLA2与T2DM患者并发血管病变症的相关指标呈显著正相关关系,对并发血管病变症具有较高的诊断价值。
Objective To explore the correlation between serum lipoprotein-associated phospholipase A2()Lp-PLA2 level and vasculopathy occurrence in patients with type 2 diabetes mellitus(T2DM).Methods 235 cases of patients with T2DM diagnosed in our hospital from September 2015 to September 2017 were retrospectively analyzed.According to the presence or absence of vascular complications,the patients were divided into non-vasculopathy group(127 cases)and asculopathy group(108 cases).The related data of 100 healthy people who were given diabetes screening were taken as normal control group.The levels of fasting plasma glucose(FPG),postprandial 2h blood glucose(2hPG),glycated hemoglobin(HbA1c),total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),interleukin(IL-6),C-reactive protein(CRP)and Lp-PLA2 were measured in each group.Pearson correlation analysis was used to analyze the correlation between Lp-PLA2 and indexes of vasculopathy complications,and the ROC curve was used to analyze its diagnostic value.Results Compared with non-vasculopathy group,the levels of FPG,2hPG,HbA1c,TC,TG,LDL-C,IL-6,CRP and Lp-PLA2 in vasculopathy group were significantly increased while the HDL-C level was significant decreased.Pearson correlation analysis showed that Lp-PLA2 was significantly positively correlated with HbA1c,IL-6 and CRP.ROC curve analysis showed that the AUC of Lp-PLA2in diagnosing T2DM with vasculopathy was 0.873.Conclusions Lp-PLA2 is significantly positively correlated with related indexes of vasculopathy in T2DM patients,and it has a high diagnostic value for concurrent vasculopathy.
