免疫球蛋白不同剂量治疗新生儿ABO溶血症对IgG1、IgG3水平的影响 被引量:9

Effect of different doses of immunoglobulin on IgG1 and IgG3 levels in the treatment of neonatal ABO hemolysis

摘要 目的:研究不同剂量免疫球蛋白治疗新生儿ABO溶血症对免疫球蛋白G亚型(IgG1、IgG3)水平的影响。方法:选取2016年9月~2020年9月在我院接受治疗的120例ABO溶血症患儿作为研究对象,按照随机数字表法将其随机分为观察组和对照组,每组60例,对照组采用常规剂量分3d注射人免疫球蛋白方案,观察组采用单次高剂量注射免疫球蛋白方案,比较两组黄疸消退时间、光疗时间、住院时间、游离抗体试验转阴率、换血发生率,比较两组治疗前后血常规[总胆红素(TBIL)、血红蛋白(Hb)、中性粒细胞百分比(Neu)]及免疫球蛋白G亚型(IgG1、IgG2、IgG3、IgG4)水平,另比较两组治疗期间不良反应发生情况。结果:观察组黄疸消退时间、光疗时间、住院时间、换血发生率显著低于对照组,差异有统计学意义。观察组治疗后TBIL、Hb、Neu显著低于治疗前及对照组。观察组治疗后IgG1、IgG3水平显著高于治疗前及对照组。观察组不良反应总发生率为16.67%与对照组15.00%比较,P>0.05。结论:单次高剂量免疫球蛋白注射可通过改善IgG1、IgG3更好的提高ABO溶血病新生儿治疗效果,临床价值较高。 Objective To study the effect of different doses of immunoglobulin on the levels of IgG1 and IgG3 in neonatal ABO hemolysis.Method 120 children with ABO hemolysis treated in our hospital from September 2016 to September 2020 were selected as the research objects and randomly divided into the observation group and the control group according to the random number table method,with 60 cases in each group,The control group was given routine dose of human immunoglobulin in three days,and the observation group was given single high dose of immunoglobulin,The jaundice regression time,phototherapy time,hospital stay,negative rate of free antibody test,exchange transfusion rate,blood routine[total bilirubin(TBIL),hemoglobin(HB),percentage of neutrophils(neu)]and immunoglobulin G subtypes(IgG1,IgG2,IgG3,IgG4)were compared between the two groups before and after treatment,The adverse reactions were compared between the two groups.Results The jaundice regression time,phototherapy time,hospitalization time and exchange transfusion rate of the observation group were significantly lower than those of the control group.TBIL,Hb,neu of the observation group after treatment were significantly lower than those before treatment and the control group.The levels of IgG1 and IgG3 in the observation group after treatment were significantly higher than those before treatment and in the control group.The total incidence of adverse reactions in the observation group was 16.67%compared with 15.00%in the control group,P>0.05.Conclusion High dose single immunoglobulin injection can improve the therapeutic effect of ABO hemolytic disease by improving IgG1 and IgG3,which has high clinical value.
