宝石能谱CT虚拟平扫技术在CT泌尿系造影检查中的应用价值探讨 被引量:3

Discussion on Application Value of Virtual Non-Contrast Scan Technique of Gemstone Spectral Imaging CT in CT Urography

摘要 目的:探讨宝石能谱(Gemstone Spectral Imaging,GSI)CT虚拟平扫技术(Virtual Non-Contrast scan,VNC)在CT泌尿系造影(CT urography,CTU)检查中的应用价值。方法:选取2019年10月~2020年10月我院泌尿外科门诊疑泌尿系梗阻病变到我科行CTU的病人80例,随机均分为A、B两组。A组行常规CTU扫描:真实平扫(True Non Contrast scan,TNC)、动脉期、实质期、延时期。B组动脉期使用GSI扫描模式,其他时相扫描均与A组相同。比较A、B两组的总有效辐射剂量、A组与B组增强三期有效辐射剂量、B组内TNC图像和VNC图像的图像质量。以同一部位的图像信噪比(Signal to Noise Ratio,SNR)和对比噪声比(Contrast to Noise Ratio,CNR)作为客观图像质量评价标准,主观图像质量评价由3名放射科医师对总体图像质量、锐利度、噪声、病灶显示情况4个方面进行双盲评估。利用SPSS软件对实验结果进行统计学分析。结果:B组总有效辐射剂量(31.39±1.74)mSv高于A组总有效辐射剂量(24.66±0.97)mSv,但B组应用VNC的增强三期总有效辐射剂量(20.18±1.56)mSv显著低于A组总有效辐射剂量;图像质量的客观评价中,TNC和VNC的SNR无统计学差异(4.82±0.55和4.69±0.44),CNR亦无统计学差异(2.62±0.46和2.50±0.37)。图像质量的主观评价中,VNC的整体图像质量、噪声水平、病灶显示情况与TNC图像比较均无统计学差异;VNC图像的锐利度不如TNC图像的锐利度,差异有统计学意义。结论:在泌尿系梗阻性病变的CTU检查中,能谱CT VNC能替代TNC,其不仅能满足临床诊断的需求,还能大大减少患者所受的辐射剂量,缩短检查时间,具有较大的临床应用价值。 Objective To explore the application value of Virtual Non-Contrast(VNC)scan technique of Gemstone Spectral Imaging(GSI)CT in CT urography(CTU).Methods Eighty patients with urinary tract obstruction from department of urology outpatient from October 2019 to October 2020 were examinated in department of radiology.They were randomly divided into two groups,A and B.Group A routine CTU scan:True Non-Contrast scan(TNC),arterial phase,parenchymal phase,delay phase.The GSI scan mode was used in the arterial phase of B group,and the other phase of B group were the same as the A group.Compare the total effective radiation dose of the A and B groups,the effective radiation dose of A group and the three enhance phases of the B group,the image quality of the TNC image and the VNC image of the B group.The Signal to Noise Ratio(SNR)and Contrast to Noise Ratio(CNR)of the same part are used as objective image quality evaluation standards.The subjective image quality is evaluated in a double-blind assessment of overall image quality,sharpness,noise,and lesion display by 3 radiologists.Statistical analysis was performed on the experimental results using SPSS software.Results The total effective radiation dose(31.39±1.74)mSv of group B was higher than the total effective radiation dose of group A(24.66±0.97)mSv,but the total effective radiation dose of three enhanced phases of group B used VNC(20.18±1.56)mSv was significantly lower than the total effective radiation dose of group A;In the objective evaluation of image quality,the SNR of TNC and VNC was not statistically different(4.82±0.55 and 4.69±0.44),CNR also there were no statistical differences(2.62±0.46 and 2.50±0.37).In the subjective evaluation of image quality,the overall image quality,noise level,lesion display of TNC images and VNC images were not statistically different;The sharpness of VNC images was not as sharp as that of TNC images,and the difference was statistically significant.Conclusion In the CTU examination of urinary obstructive diseases,the VNC of GSI CT can replace TNC,which can not only meet the needs of clinical diagnosis,but also greatly reduce the radiation dose of patients,shorten the examination time,and have a large clinical Value.
