摘要
目的:探究氨溴索注射液对肺结核合并糖尿病患者Toll样受体4(TLR4)/核因子-κB(NF-κB)信号通路表达的影响。方法:选取2018年4月~2020年2月期间我院收治的肺结核合并糖尿病患者113例,考虑最终脱落5例,最终确定样本量共108例。根据患者入院顺序,并按机数字表法将其分为治疗组(54例)及对照组(54例)。两组均给予规范抗结核药物治疗,在此基础上,治疗组强化期加予盐酸氨溴索注射液静脉滴注,继续期改为盐酸氨溴索口服液口服。于治疗前后检测两组患者TLR4/NF-κB信号通路信号通路相关因子、相关炎症反应因子以及T淋巴细胞亚群水平的差异,并记录两组治疗后的病灶吸收、空洞变化以及药物不良反应发生情况。结果:经有效治疗后,两组TLR4、NF-κB均呈降低趋势,且治疗组较对照组更低;两组IL-6、IL-1β均呈降低趋势,且治疗组较对照组更低;两组INF-γ均呈升高趋势,且治疗组较对照组更高;两组CD8^(+)均呈降低趋势,且治疗组较对照组更低;两组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均呈升高趋势,且治疗组较对照组更高;同时,治疗组临床疗效高于对照组,且治疗组闭合率亦明显高于对照组。结论:在规范抗结核药物治疗基础上,氨溴索注射液辅助治疗肺结核合并糖尿病具有一定疗效,其机制可能与抑制TLR4/NF-κB信号通路的活化、炎性因子释放以及提高机体免疫功能有关。
Objective To explore the effect of ambroxol injection on the expression of Toll-like receptor 4(TLR4)/nuclear factor-κB(NF-κB)signaling pathway in patients with pulmonary tuberculosis and diabetes.Methods A total of 113 patients with pulmonary tuberculosis complicated with diabetes mellitus admitted to our hospital from April 2018 to February 2020 were selected,and 5 cases were finally considered to fall off,and a total of 108 cases were finally determined.According to the order of admission,the patients were divided into treatment group(54 cases)and control group(54 cases).Both groups were given standard anti tuberculosis drug treatment.On this basis,the treatment group was given ambroxol hydrochloride injection intravenous drip in the intensive phase,and was changed to ambroxol hydrochloride oral liquid in the continuous period.The levels of TLR4/NF-κB signaling pathway related factors,inflammatory response factors and T lymphocyte subsets were detected before and after treatment,and the lesion absorption,cavity changes and adverse drug reactions were recorded.Results After effective treatment,the two groups of TLR4 and NF-κB showed a decreasing trend,and the treatment group was lower than the control group;both groups of IL-6,IL-1β showed a decreasing trend,and the treatment group It was lower than the control group;the two groups of INF-γ showed an increasing trend,and the treatment group was higher than the control group;the two groups of CD8^(+)showed a decreasing trend,and the treatment group was compared with the control group lower;the two groups of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)showed an upward trend,and the treatment group was higher than the control group;at the same time,the clinical efficacy of the treatment group was significantly higher than that of the control group,and the closure rate of the treatment group was also significantly higher than that of the control group.Conclusion On the basis of standard anti-tuberculosis drug treatment,ambroxol injection has a certain curative effect in adjuvant treatment of tuberculosis complicated with diabetes.Its mechanism may be related to inhibiting the activation of TLR4/NF-κB signaling pathway,releasing inflammatory factors and improving the body’s immune function.
