摘要
目的探讨血清vaspin水平与慢性心力衰竭分级的相关性,血清vaspin在慢性心力衰竭进展中的作用和机制。方法选取慢性心衰(chronic heart failure,CHF)患者83例为实验组;选取同期80例健康体检者为对照组。将CHF组按NYHA分级,分为Ⅱ、Ⅲ、Ⅳ级。测定血清vaspin和TNF-α水平,同时测定实验对象的空腹血糖、血脂、CRP,并记录血压、身高、体重、BMI。给予心脏三维彩超检查,记录心脏左室射血分数(LVEF)。结果慢性心力衰竭组与健康对照组比较,CRP、LVEF水平差异明显(P<0.05);慢性心力衰竭组血清vaspin的表达水平低于健康对照组(P<0.05),且心衰程度越重血清vaspin水平越低(P<0.05)。慢性心衰组TNF-α水平比健康对照组明显增高(P<0.01),且心衰程度越重血清TNF-α水平越高(P<0.05)。回归分析表明,慢性心力衰竭组血清vaspin的水平与BMI、LVEF、TG呈正相关,与TNF-α呈负相关。结论血清vaspin对心功能可能起保护作用。vaspin有可能通过抑制炎症而发挥其心脏保护的作用,推测其参与血脂代谢紊乱。
Objective To investigate the different grade of chronic heart failure,and the association of serum vaspin levels with inflammatory mediator,in order to clarify the role and mechanism of serum vaspin in chronic heart failure’s occurence and development.Methods A total of 83 patients with chronic heart failure(CHF)were selected as the experimental group;and 80 healthy patients during the same period as the control group.The CHF group was classified according to NYHA and divided intoⅡ,Ⅲ,andⅣgrade.Serum vaspin and TNF-αlevels were measured,and fasting blood glucose,blood lipids,and CRP were measured at the same time,and blood pressure,height,weight,and BMI were recorded.The heart was examined by a three-dimensional color Doppler ultrasound and the left ventricular ejection fraction(LVEF)was recorded.Results The levels of CRP and LVEF showed significant changes between the chronic heart failure group and the healthy control group(P<0.05).The serum vaspin expression level in the chronic heart failure group was lower than that in the healthy control group(P<0.05),and the levels of CRP and LVEF showed a severe-dependent manner in the chronic heart failure group.The TNF-αlevel in the chronic heart failure group was significantly higher than that in the healthy control group(P<0.01),and the TNF-αlevel also showed a severe-dependent manner in the chronic heart failure group.Regression analysis showed that serum vaspin level in chronic heart failure group was positively correlated with BMI,LVEF,TG,and negatively correlated with TNF-α.Conclusion Illustrating vaspin may have the potential protective effect of cardiac function;serum vaspin may have the effect of inhibiting the inflammatory to protect the heart,and vaspin may involve in lipid metaboliam disorders.
