摘要
目的回顾性分析肿块周围低密度晕环对乳腺癌的诊断价值及临床意义。方法收集2017年1月~2019年12月在哈尔滨医科大学附属第一医院就诊的282例(恶性241例,良性41例)乳腺肿块患者的临床、病理及影像资料,比较良恶性肿块周围“低密度晕环”的差异;并分析乳腺癌周围“低密度晕环”的特征及其与相关分子生物学标志物的关系。结果(1)良性肿块周围“低密度晕环”平均宽度(3.67±1.16)mm;恶性肿块周围“低密度晕环”平均宽度(14.45±4.36)mm,且恶性肿块周围“低密度晕环”不规则、不清晰、不完整的发生率明显高于良性肿快,差异有统计学意义(P<0.001)。(2)“低密度晕环”完整性与人表皮生长因子受体-2(human epidermal growth factor receptor 2,HER2)、表皮生长因子受体(epidermal growth factor receptor,EGFR)阳性表达率呈负相关;“低密度晕环”的规则性和清晰度与CK5/6和EGFR阳性表达率呈负相关(P<0.05)。结论“低密度晕环”是乳腺癌的一个重要X线征象。低密度晕环的完整性、规则性、清晰度等特征与HER-2、CK5/6、EGFR的表达存在一定的相关性。
Objective To retrospectively analyze the diagnostic value of hypodense halo ring around the mass in breast cancer and its clinical significance.Methods Clinicopathological and imaging data of 282 patients(241 malignant and 41 benign)with breast mass who were treated in the First Affiliated Hospital of Harbin Medical University from January 2017 to December 2019 were collected.The difference of hypodense halo ring around benign and malignant masses were compared.The relationship between the characteristics of hypodense halo ring and molecular biomarkers around breast cancer was analyzed.Results①The average width of the hypodense halo ring around the benign mass was(3.67±1.16)mm,the average width of the hypodense halo ring around the malignant mass was(14.45±4.36)mm,and the hypodense halo ring around the malignant mass was counted to be irregular,unclear and incomplete.The incidence was significantly higher than that of benign tumors,and the difference was significant(P<0.01).②The integrity of the hypodense halo ring was negatively correlated with the positive expression rate of human epidermal growth factor receptor 2(HER-2)and epidermal growth factor receptor(EGFR);the regularity and clarity of the hypodense halo ring were negatively correlated with the positive expression rate of CK5/6 and EGFR,with statistical significance(P<0.05).Conclusion Hypodense halo ring is an important X-ray sign of breast cancer.The integrity,regularity,clarity and other characteristics of low density halo ring are related to the expression of HER-2,CK5/6 and EGFR.
