摘要
目的分析对比神经内镜与显微镜应用于急性脑出血破入脑室患者手术效果及颅内感染的影响。方法选取80例2019年1月—2020年2月安阳市人民医院收治的急性脑出血破入脑室患者,在显微镜下实施脑实质血肿清除和脑室钻孔引流手术者为对照组,在神经内镜下实施脑实质血肿清除和脑室钻孔引流手术者为研究组,比较两组手术情况、血肿清除率以及颅内感染情况。结果研究组术后带管时间、术后7d GCS评分、脑脊液引流时间与对照组比较差异有统计学意义(P<0.05)。研究组颅内感染发生率为5.0%,对照组颅内感染发生率为20.00%,P<0.05,差异有统计学意义。研究组术后1d、7d的脑室血肿清除率分别为(58.96±5.75)%和(72.84±9.69)%,与对照组比较均明显较高(P<0.05)。研究组术后7d的脑实质血肿清除率(87.45±8.06)%,高于对照组的(80.37±7.64)%,P<0.05。两组不良反应差异无统计学意义(P>0.05)。结论与显微镜相比,急性脑出血破入脑室患者在神经内镜下手术的效果显著较优,可显著提升患者的脑室和脑实质血肿清除率,缩短患者脑脊液引流时间,降低患者颅内感染风险,安全性高,可推广应用。
Objective To analyze and compare the effects of neuroendoscopy and microscopy on the surgical results of patients with acute intracerebral hemorrhage.Method Chose 80 cases from January 2019 to February 2020 treated in our hospital during the period of patients with acute cerebral hemorrhage broken into ventricles,and the control group under the microscope the parenchymal hematoma removal and ventricle drainage drilling operation,the team in neural endoscopic parenchymal hematoma removal and ventricle drainage drilling operation,comparing two groups of operation condition and hematoma removal effect.Results Compared with the control group,the time of postoperative catheter insertion,7 d GCS score and CSF drainage time were all P<0.05.The incidence of intracranial infection was 5.0%in the study group and 20.00%in the control group,P<0.05,the difference was statistically significant.The removal rates of ventricular hematoma in the study group were(58.96±5.75)%and(72.84±9.69)%,respectively,1 d and 7 d after operation,which were significantly higher than those in the control group(P<0.05).The clearance rate of cerebral parenchymal hematoma(87.45±8.06)%7 days after operation in the study group was higher than that in the control group(80.37±7.64)%,P<0.05.There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Compared with microscopy,neuroendoscopic surgery for patients with acute cerebral hemorrhage into the ventricle has a significantly better effect,which can significantly improve the removal rate of ventricular and parenchymal hematoma in patients,shorten the time of cerebrospinal fluid drainage in patients,reduce the risk of intracranial infection in patients with high safety,and can be popularized.
