摘要
目的探讨异种脱细胞真皮基质敷料(acellular dermal matrix dressing,ADM)联合Meek植皮治疗深度烧伤的效果。方法选取安阳市人民医院2019年10月至2021年10月收治的严重烧伤患者34例,随机分为两组各17例,观察组行ADM联合Meek治疗,对照组行ADM联合自体邮票植皮,比较两组各项手术指标、治疗效果与不良反应。结果观察组手术面积(34.06±5.17)%体表总面积(TBSA)与对照组(31.72±6.39)%TBSA无明显差异(t=1.174,P>0.05);观察组手术时间(121.84±32.95)min短于对照组(157.96±41.83)min(t=2.797,P=0.009);观察组补充植皮次数(1.27±0.21)次少于对照组(2.84±0.58)次(t=10.494,P<0.001);观察组皮片成活率(93.14±3.48)%高于对照组(86.32±3.91)%,皮片融合时间(10.36±2.18)d低于对照组(15.68±2.84)d(t=5.372、6.127,均P<0.001);观察组不良反应发生率(11.76%)与对照组(35.29%)无明显差异(χ^(2)=2.615,P>0.05)。结论ADM联合Meek植皮治疗深度烧伤可缩短手术与术后恢复时间,提高皮片成活率。
Objective To investigate the effect of xenoacellular dermal matrix dressing(ADM)combined with Meek skin grafting in the treatment of deep burn.Methods A total of 34 severe burn patients admitted from Oct 2019 to Oct 2021 were selected and randomly divided into two groups with 17 cases in each group,The observation group was treated with ADM combined with Meek,and the control group was treated with ADM combined with autologous stamp skin grafting,the surgical indicators,therapeutic effects and adverse reactions of the two groups were compared.Results There was no significant difference in surgical area between the observation group(34.06±5.17)%TBSA and the control group(31.72±6.39)%TBSA(t=1.174,P>0.05);The operation time of observation group(121.84±32.95)min was shorter than that of control group(157.96±41.83)min(t=2.797,P=0.009);The number of supplemental skin grafts in the observation group(1.27±0.21)was less than that in the control group(2.84±0.58)(t=10.494,P<0.001);The skin survival rate in the observation group(93.14±3.48)%was higher than that in the control group(86.32±3.91)%,and the skin fusion time(10.36±2.18)d was lower than that in the control group(15.68±2.84)d(t=5.372,6.127,P<0.001);There was no significant difference in the incidence of adverse reactions of observation group(11.76%)and the control group(35.29%)(χ^(2)=2.615,P>0.05).Conclusion ADM combined with Meek skin grafting can shorten the operation and postoperative recovery time and improve the survival rate of skin slices in deep burn patients.
