摘要
目的研究儿童替牙期错位伴继发性咬合创伤牙齿采用正畸-牙周联合治疗对局部组织的疗效。方法选取信阳市中心医院2018年2月—2020年1月收治的88例替牙期儿童。将错位伴继发性咬合创伤牙齿的44例儿童患者作为研究组,将错位但是不伴有继发性咬合创伤的44例儿童作为对照组。牙周状态评估之后再进行牙周-正畸联合治疗,对比两组患者治疗之前以及治疗之后错位牙的几组指标,分别为牙周袋探针深度(PD)、牙龈指数(GI)、出血指数(BI)以及菌斑指数(PI)。结果研究组与对照组经过治疗后,错位牙的咬合、邻牙邻接关系均恢复正常,错位牙排列也恢复正常,研究组牙龈探诊也没有看到明显出血,TM在正常范围之内,错位牙稳固性显著改善;错位牙的牙周指标PI、BI、GI、PD方面研究组治疗之前均高于对照组(P<0.05),两组治疗后错位牙的牙周状况临床指标与治疗前比较均显著降低(P<0.05),且研究组与对照组PD、GI、BI、PI等指标对比(P>0.05)。结论针对于替牙期出现错位伴继发性咬合创伤牙齿患儿应该予以高度重视,在早期干预之中,正畸-牙周联合治疗是有效手段,具有去除咬合创伤与排齐牙齿、作用于患儿牙周组织的改建和修复、有利于患儿健康成长的明显优势,临床推广意义重大。
Objective To study the therapeutic effect of orthodonti-periodontal combination on local tooth tissue in children with dislocation combined with secondary occlusal trauma during the dental replacement stage.Methods A total of 88 children with dental replacement stage admitted to our hospital from 2018 to 2019 were selected.According to the selection criteria,we divided the patients into two groups:study group(44 cases),the patients were children with malposition accompanied by secondary occlusal trauma;The control group(44 cases)was a group of children with dislocation but no secondary disease or trauma.The periodontal status assessment was followed by periodontium-orthodontic combined treatment,and several indexes of the two groups of patients with misplaced teeth before and after treatment were compared,including periodontal pocket probe depth(PD),gingival index(GI),bleeding index(BI)and plaque index(PI).Results In the study group and the control group,after treatment,the occlusal of the dislocated teeth,the adjacent teeth adjacent relationship were restored to normal,the dislocated teeth were also restored to normal,gingival probing in the study group also did not see obvious bleeding,TM was within the normal range,the stability of the dislocated teeth was significantly improved;The periodontal indexes PI,BI,GI and PD in the study group were higher than those in the control group before treatment(P<0.05).After treatment,the periodontal clinical indexes of the two groups were significantly decreased compared with those before treatment(P<0.05),and the PD,GI,BI,PI and other indexes in the study group were compared with those in the control group(P>0.05).Conclusion for phase mismatch for teeth with secondary occlusal trauma tooth should be attached great importance to children,in the early intervention,orthodontic-periodontal combination therapy is an effective means of removal of occlusal trauma and aligned teeth,applies to children of periodontal tissue reconstruction and repair,and is beneficial to the healthy growth of children with obvious advantages,clinical popularization is of great significance.
