摘要
目的探讨回收式自体血回输对腰椎体间融合内固定术患者凝血功能及血小板的影响。方法选取芜湖市第一人民医院自2016年4月至2021年10月收治的99例接受腰椎体间融合内固定术患者为研究对象。根据输血方式不同,将采用同种异体血输入治疗的47例患者纳入A组,将采用回收式自体血回输治疗的52例患者纳入B组。比较两组术中总出血量和输血量、输血前后凝血功能指标和血小板水平,以及输血后并发症发生情况。结果A组和B组术中总出血量差异无统计学意义(P>0.05)。B组输血量低于A组,差异有统计学意义(P<0.05)。输血后,A组和B组D-二聚体均高于输血前,且A组高于B组,差异有统计学意义(P<0.05)。输血后,A组和B组血小板均低于输血前,差异有统计学意义(P<0.05)。B组并发症发生率低于A组,差异有统计学意义(P<0.05)。结论与同种异体血输入比较,在腰椎体间融合内固定术中采用回收式自体血回输可取得相似治疗效果,同时能够减少输血量和降低并发症发生风险。
Objective To investigate the effects of autologous blood transfusion on coagulation function and platelet in patients with lumbar interbody fusion and internal fixation.Methods A total of 99 patients with lumbar interbody fusion and internal fixation were selected from Wuhu NO.1 People′s Hospital from April 2016 to October 2021.According to different transfusion methods,47 patients treated with allogeneic blood transfusion were included in group A,and 52 patients treated with autologous blood transfusion were included in group B.Total intraoperative blood loss and transfusion volume,coagulation function indexes and platelet levels before and after transfusion,and the incidence of complications after transfusion were compared between the two groups.Results There was no significant difference in total intraoperative blood loss between group A and group B(P>0.05).The amount of blood transfusion in group B was lower than that in group A,the difference was statistically significant(P<0.05).After transfusion,D-dimer in group A and group B was higher than before,and group A was higher than group B,the difference was statistically significant(P<0.05).After transfusion,platelets in group A and group B were lower than before,the difference was statistically significant(P<0.05).The complication rate of group B was lower than that of group A,and the difference was statistically significant(P<0.05).Conclusion Compared with allogeneic blood transfusion,autologous blood transfusion can achieve similar therapeutic effect in patients with lumbar interbody fusion and internal fixation and reduce the amount of blood transfusion and the risk of complications.
