摘要
目的:探讨基于冠状动脉CT血管成像指南在256排螺旋CT个体化对比剂精准定量流速应用的优化方法。方法:将在某医院接受256排CT冠状动脉检查的患者分2组。A组38例,B组32例。A组应用(去脂体质量/50+0.4)得出碘流率,再由碘流率/对比剂浓度定量计算出对比剂流速,应用该流速进行扫描;B组应用经验流速的方法扫描(体质量≤50 kg使用流速4.0,50 kg<体质量≤70 kg使用流速4.5,体质量>70 kg使用流速5.0),其余扫描方案相同。测量每个病例的CTA图像上冠状动脉根部及主要分支近端CT值及记录每个患者使用对比剂总量,并进行两组数据方差分析。结果:A组使用的对比剂总量少于B组,且差异有统计学意义(P<0.05);A组图像信噪比小于B组,且差异有统计学意义(P<0.05)。结论:应用碘流率(去脂体质量/50+0.4)可以精准定量计算流速方法可以在图像质量保持不变的情况下减少对比剂用量。
Objective: To explore the optimization method of accurate quantitative flow velocity application of 256-slice spiral CT individualized contrast medium based on coronary CT angiography guidelines. Methods: 256-slice CT coronary angiography was divided into two groups. There were 38 patients in group A and 32 patients in group B. In group A, the iodine flow rate was obtained using(fat-free mass/50 + 0.4), and then the contrast medium flow rate was quantitatively calculated from the iodine flow rate/contrast medium concentration, which was used for scanning. In group B, the empirical flow rate method was used for scanning(weight ≤ 50 kg using flow rate 4.0, 50 kg < weight ≤ 70 kg using flow rate 4.5, and weight > 70 kg using flow rate 5.0), and the other scanning protocols were the same. The CT values of the coronary root and the main branches on the CTA images of each case were measured and the total amount of contrast medium used by each patient was recorded, and the analysis of variance of the two groups of data was performed. Results: The total amount of contrast medium used in group A was less than that in group B, and the difference was statistically significant(P < 0.05). The image signal-to-noise ratio in group A was less than that in group B, and the difference was statistically significant(P < 0.05). Conclusion: The quantitative calculation of flow rate with iodine flow rate(fat-free mass/50 + 0.4) can reduce the amount of contrast medium when the image quality remains unchanged.
